Speaking at a press conference on April 23, an air force representative said they “sincerely apologize for the concerns caused by the 2021 accident,” and confirmed that one of the pilots involved had been suspended from flying, received severe disciplinary action, and had left the military.

The apology came after a report released on April 22 by the South Korean State Audit and Inspection Commission. The report stated that two F-15K fighter jets collided while flying in formation near Daegu in December 2021, due to the pilots independently performing unplanned maneuvers to take personal photos and videos .
According to the audit findings, a co-pilot attempted to record footage to mark his final flight with the unit. He independently increased altitude and tilted the aircraft to improve the camera angle, while another pilot on the lead aircraft was also filming.
As the two aircraft approached, both crews attempted to maneuver to avoid a collision, but the tail of the rear aircraft struck the wing of the lead aircraft. The incident resulted in approximately 880 million won (equivalent to $600,000 USD) in damage, but there were no casualties.
The auditing agency determined that the co-pilot was primarily responsible, and also pointed out shortcomings in the air force's control over aerial filming at the time. The report required the pilot involved to reimburse approximately one-tenth of the repair costs.
The South Korean Air Force said it is implementing stricter flight safety regulations and controls on video recording to prevent similar incidents in the future.
