Hunter with a Scalpel is a Korean psychological crime film revolving around Seo Se-Hyun (Park Ju-Hyun), a genius forensic doctor with antisocial personality disorder. During an autopsy, she discovers the signature killing method of her father - Yoon Jo-Kyun (Park Yong Woo) - a serial killer who was thought to have died 20 years ago. This forces her to embark on a dangerous hunt to cover up her father's criminal past.

Se-Hyun must deal with Jo-Kyun's return when a series of new cases appear with the same "signature" of murder as her father. Instead of cooperating with the police, she tries to approach Jo-Kyun first to protect her identity and career. However, the confrontation between father and daughter gradually turns into a bloody mind game, where each side tries to trap and manipulate the other.

The toxic father-daughter relationship between Se-Hyun and Jo-Kyun goes beyond blood. The two are forced to hunt each other down to survive: Se-Hyun uses her wits to set traps to stop the past, while Jo-Kyun seeks to turn his daughter into an accomplice.
