"Dissecting" the problem, finding solutions to the shortage problem

To dissect the problem and find a solution to the shortage of medical supplies and biological chemicals in Ha Tinh that has existed for many months, Nguoi Dua Tin had an interview with Mr. Vo Viet Quang - Head of Health Insurance Appraisal Department, Social Insurance of Ha Tinh province.

Mr. Quang said that the locality has more than 1.54 million people participating in health insurance (coverage rate reached 94%). In March 2023 alone, more than 175,000 people participating in health insurance went to medical facilities for examination and treatment. The lack of equipment, medical supplies, chemicals, and biological products directly affects health insurance participants and the quality of medical examination and treatment.

The monitoring results of the Ha Tinh Provincial Social Insurance on April 4, 2023 on the supply of drugs, equipment, and chemicals in health insurance examination and treatment showed that there was a complete shortage of 140 types of drugs at 12 health insurance examination and treatment facilities. Most of them were traditional medicine and medicinal herbs. These drugs were centrally bid but no bidders submitted bid documents or bidders bid but did not win the bid due to price overruns.

With a large number of patients, Ha Tinh General Hospital is currently facing a serious shortage of medical supplies and biological chemicals for examination and treatment.

There is a partial shortage of 59 types of drugs at 7 health insurance examination and treatment facilities. These drugs were not awarded the bid, however, the examination and treatment facilities still have inventory according to the old bidding results or can switch to using other types instead.

There is a complete shortage of 59 types of medical supplies and chemicals at 5 medical examination and treatment facilities due to the lack of new bidding results, the contractor does not agree to extend the implementation period of the centralized procurement framework agreement according to the policy of the Provincial People's Committee in Document No. 187/UBND-TH1 on extending the implementation period of the Centralized Procurement Framework Agreement for Medical Supplies, Chemicals, and Biological Products of the Health Sector in 2020 or agrees to extend but there is no source of goods. There is a partial shortage of 78 types of medical supplies and chemicals at 4 medical examination and treatment facilities due to the contractor not agreeing to sign the appendix to extend the implementation period of the Centralized Procurement Framework Agreement for Medical Supplies, Chemicals, and Biological Products.

In the recent announcement of the Conclusion of the People's Committee of Ha Tinh province, dated April 27, 2023, it was assigned: "The Department of Health is responsible for inspecting and directing medical facilities to organize timely procurement in accordance with the provisions of law. The Director of the Department of Health is responsible to the Provincial People's Committee and the Chairman of the Provincial People's Committee, absolutely not allowing a shortage of medical supplies, chemicals, and biological products to serve medical examination and treatment at medical facilities in the province".

Every day, every hour, patients participating in health insurance are at a disadvantage in terms of benefits.

The announcement of the Provincial People's Committee is like that, while the framework agreement has expired on April 28, 2023, hospitals cannot sign self-procurement contracts. Because hospitals self-procure (packages under 100 million VND) also violate because the missing supplies, chemicals, and biological products are all on the list of centralized procurement.

“The direction of the Provincial People's Committee has not provided a solution to the shortage of supplies, chemicals, and biological products that has been going on for more than 9 months. The words "in accordance with regulations, in accordance with the law, and responsible" haunt us when faced with specific matters in practice next to general documents,” a hospital leader sighed after receiving the above notice.

There needs to be a specific plan for officials to dare to work for the people.

Recently, the People's Committee of Ha Tinh province and relevant agencies have proposed many solutions to overcome the difficulties of the shortage of medical supplies, chemicals and biological products at medical examination and treatment facilities. As an immediate solution, the People's Committee of the province has agreed to allow units to organize their own procurement to ensure medical supplies, chemicals and biological products for medical examination and treatment at medical facilities.

The framework agreement has now expired on April 28, and hospitals cannot sign contracts for self-purchasing. However, if hospitals now self-procure (a package of less than 100 million VND), they will still violate the law because the materials, chemicals, and biological products in short supply are all on the list of centralized procurement.

Ha Tinh needs to soon have a specific solution to "solve" the problem of shortage of medical supplies and biological chemicals.

According to a leader of the Ha Tinh Department of Finance, all of the above problems have not been resolved because of some articles in Decision 172/QD-UBND/14-01-2019 of the Provincial People's Committee on promulgating the list of assets for centralized procurement in the province. In particular, for medical equipment serving medical examination and treatment with a price of 100 million VND/equipment or more, centralized bidding must be organized. Currently, the Department of Finance is completing a draft to amend some articles in Decision 172 to soon submit to the leaders of the Provincial People's Committee.

Answering the reporter's question, should we propose to abolish bidding and centralized procurement and give autonomy to departments, branches, and medical examination and treatment units? This leader said that it is impossible to abolish bidding and centralized procurement because it would be contrary to the provisions of the Law and Decree on management and use of public assets.

This person also explained that centralized procurement will organize bidding in large quantities, so it is possible to select large, capable contractors, thereby ensuring the quality of goods, warranty and maintenance regimes; the purchase price can approach the wholesale price. Centralized procurement conducts procurement in the form of open bidding, thereby increasing publicity and transparency in the procurement and use of State assets. At the same time, inspection and supervision of units assigned to carry out centralized procurement are more convenient than inspection and supervision on a large scale.

On the other hand, instead of units having to organize bidding many times, many separate rounds, which is very time-consuming and labor-intensive, affecting professional work. Organizing centralized bidding reduces time, costs, and human resources for organizing bidding, helping units focus more on their professional work; enhancing professionalism in bidding, contributing to increasing economic efficiency and saving for the State budget. Limiting the situation of large price differences in medical supplies and chemicals, leading to uneven prices of medical examination and treatment services at public medical facilities, especially small medical facilities, remote areas, thereby indirectly supporting people. In addition, the unified price of goods in the area is very convenient for units to make payments and settlements.

Patients participating in health insurance "suffer double losses" when they have to pay many times the cost of normal treatment.

“In fact, medical facilities do not want to do it (organize bidding - PV) because they are professionals. Centralized bidding is necessary to ensure compliance with legal regulations on management and use of public assets. What needs to be adjusted here is that some provisions in Decision 172 of the Provincial People's Committee are no longer suitable for reality, and need to be introduced to solve the immediate problem. The Department of Finance has also proposed in the draft to give autonomy in purchasing to units but set specific purchasing norms (under 200 or 300 million VND/package),” he said.

“Whatever is done, the most important thing is to have specific instructions and plans, not general guidance documents. Only with specific plans will officials dare to work for the people,” a leader of the provincial Social Insurance emphasized.
