On November 20, 2023, a pregnant woman named NTH in Phu Tho gave birth to a baby girl at 24 weeks of pregnancy. The baby weighed only 550 grams, did not cry at birth, had a purple body, and had very poor reflexes. Doctors at Phu Tho Maternity and Pediatrics Hospital placed an emergency endotracheal tube in the delivery room, provided ventilation, warmth, and treatment.
The first days of a premature, underweight baby girl in Phu Tho.
According to Dr. Nguyen Duc Hau, Head of the Department of Neonatology, for extremely premature and low birth weight babies, the fight to save their lives is extremely difficult with a very cautious prognosis.
Previously, nationwide, there was only one case of twin babies, born prematurely at 24 weeks, successfully kept alive at the Central Maternity Hospital in 2015.
"We determined that this journey would be very difficult, especially the treatment and care process needed to be meticulous and careful. Doctors had to consider each type of medicine given, not to exceed or lack each ml of nutritional fluid, ensure temperature and humidity to avoid hypothermia and dehydration, and always closely monitor infection control issues...", Dr. Hau said.
In the first days after birth, the baby's respiratory failure increased, with episodes of cyanosis, bradycardia, acid-base disorders, etc., and was discovered by doctors, who consulted with the entire department even late at night, and applied an active treatment regimen. The baby continued to use a ventilator with very high indexes, used vasopressors, blood transfusions, plasma transfusions, Albumin, acid-base compensation, treatment of electrolyte disorders, and phototherapy for jaundice.
After 7 days of mechanical ventilation, the child's condition improved, and the ventilator index gradually decreased. On the 17th day of treatment, the child was removed from the endotracheal tube, put on non-invasive ventilation, and started to practice feeding, only 1 ml per meal. During the care process, the doctors and nurses were very careful and meticulous because the child was very young, had very thin skin, and very small and weak veins.
The child's recovery process after a long period of treatment and care by doctors and nurses.
On the 35th day of treatment, the child's weight reached 1000 grams, he was fed milk through a tube with about 18ml/meal, had his intravenous feeding stopped, and was given breathing support. The results of the child's medical screening were within normal limits, all newborn reflexes were good, and the retina was mature.
After 60 days of treatment, the child was switched to oxygen therapy, at which point the child's weight reached 1,500 grams. Tests to evaluate liver and kidney function were within normal limits, screening results for the eyes, brain, heart, and newborn reflexes were good. With these positive developments, the child was transplanted to the mother and the Kangaroo method was performed.
By the 80th day of treatment, the child was completely able to breathe on his own, drank milk well at about 50ml/meal, had good reflexes and weighed 2200 grams.
On February 23, after 93 days of treatment, care, and nurturing, the baby weighed 2,500 grams, breastfed well, was in stable health, and was discharged from the hospital.
"To minimize the risk of premature birth and the risks caused by premature birth, doctors recommend that mothers need to take good care of their pregnancy, develop a balanced and reasonable diet, and have regular pregnancy check-ups according to the doctor's instructions to promptly detect and control infections, blood pressure and other diseases, to avoid affecting the fetus," Dr. Hau advised.
