Successful emergency treatment of a patient with a perforated stomach in Con Dao special zone

On the afternoon of October 4, doctors from Binh Dan Hospital (HCMC) who are rotating to work at Con Dao Military-Civilian Medical Center, along with the on-site team, successfully performed emergency surgery on a 15-year-old student with a perforated stomach ulcer, helping the patient overcome the critical stage.

Dr. Nguyen Thi Thanh Truc (Binh Dan Hospital) directly performed anesthesia, Dr. Vu Khuong An (Binh Dan Hospital) was the main surgeon, along with the medical team and doctors of Con Dao Military-Civilian Medical Center, were performing endoscopic surgery on the patient.

Patient D.PT, 15 years old, a 10th grade student in Con Dao, was admitted to the emergency room with epigastric pain radiating to the right iliac fossa, and severe pain after eating. The patient had a history of gastric ulcer diagnosed by endoscopy 3 years ago but did not return for a follow-up visit as scheduled by the doctor.

X-ray results showed a right subphrenic air crescent, suggesting a diagnosis of hollow viscus perforation. Abdominal CT scan without contrast showed free air in the abdominal cavity with an image of gastric pyloric antrum perforation, little abdominal fluid, and mesenteric fat infiltration.

Immediately after the diagnosis, the doctors connected to an online consultation and asked for opinions from the leaders of Binh Dan Hospital. The experts unanimously diagnosed peritonitis due to a perforated hollow organ, indicated emergency surgery, and agreed to let the doctors who were rotating to work at Con Dao perform the surgery.

The surgical team included Dr. Vu Khuong An (Binh Dan Hospital), Deputy Head of the Department of Line Command and Scientific Research as the main surgeon, and Dr. Nguyen Thi Thanh Truc (Binh Dan Hospital), Deputy Head of the Department of Anesthesia and Resuscitation 1, in charge of anesthesia. Also participating in the surgical team were Dr. Nguyen Van Tai (Hung Vuong Hospital); Dr. Le Uy Phuong (Trauma and Orthopedics Hospital).

The surgery went smoothly in 60 minutes. The surgical results were as per the preoperative diagnosis, and the patient was stable after surgery.

Early detection and treatment while the perforation was still localized helped the patient avoid serious complications such as diffuse peritonitis, sepsis or septic shock. After surgery, the patient recovered well, continued antibiotic treatment and is expected to be discharged in the next few days.

Since September, the Ho Chi Minh City Department of Health has sent two batches of doctors to work at the Con Dao Military-Civilian Medical Center.

According to the Board of Directors of Binh Dan Hospital, after more than 1 month of rotating doctors to Con Dao, the examination, treatment and surgical procedures at Con Dao Military-Civilian Medical Center have stabilized and are increasingly receiving the trust of local people.

In this second phase, on the first working day, doctors examined 133 outpatients and 3 inpatients; provided emergency care for 8 cases, including 2 cases of emergency surgery.
