The day before (July 22), JW Hospital received a case of deep tissue necrosis and abscesses in the entire muscle area. Medical history revealed that in 2019, Ms. NTH (29 years old, Singaporean nationality) had injected about 1000ml of filler at an unlicensed spa, then continued to perform fat grafting to plump her buttocks in 2021. However, after a period of filler injection, her buttocks and thighs became concave and convex, and the muscle tissue was loose, causing her to lose confidence in daily activities and have difficulty walking.
Dr. Nguyen Phan Tu Dung said that through clinical examination and diagnosis by MRI and 5D color ultrasound, the filler has penetrated deep into the muscle tissue layer, causing necrosis spreading from the buttocks to the thighs, but the patient was completely unaware, leading to late treatment, making the abscess extremely serious.
"The cause was determined to be due to the technique of injecting filler in the wrong position, through the muscle capsule, into the muscle layer, injecting too much, in all positions causing the filler to penetrate deep everywhere, causing infection and necrosis deep into the muscle tissue," said Dr. Dung.
Because the filler had spread throughout the positions, creating a honeycomb structure that was difficult to identify, Dr. Dung indicated that it was necessary to perform urgent filler removal surgery, and at the same time ordered the application of AI ultrasound technology throughout the surgery.

After the surgery, nearly 3000 ml of necrotic fluid mixture including filler, necrotic tissue and blood was aspirated. The patient will continue to be placed on the VAC negative pressure suction machine for 5 days, and receive care to recover. After 6 months - 1 year, the patient can have fat grafting to fill the buttocks.
"Only perform filler injections at licensed medical facilities with well-trained specialists. Do not believe in cheap advertising, injections at home, or those of unclear origin. When unusual signs appear after injection (prolonged pain, swelling, redness, deformity, loss of sensation, etc.), go to a specialized hospital for timely examination and treatment," Dr. Dung advised.
