On the morning of June 13, Dr. Dang Quoc Quan - Director of Hoc Mon Regional General Hospital (HCMC) - said that the hospital's stroke unit had just received and successfully treated a difficult stroke case. This was the first case to be treated with thrombolysis (rt-PA) with the support of People's Hospital 115.
According to Dr. Quan, this is an important milestone, demonstrating the unit's growing professional capacity, while opening up expectations for improving the quality of stroke treatment locally, as well as improving the quality of stroke treatment in the Northwest region of the city.
Previously, at 5:25 p.m. on June 12, the patient (female, 57 years old) was hospitalized due to weakness in the right side of the body, starting in the first hour.
Clinical examination showed the patient had right hemiplegia (muscle strength 0/5), right central facial paralysis, difficulty speaking, and aphasia. The patient had a history of hypertension, atrial fibrillation, and was taking medication as prescribed.
Upon admission, the stroke alert protocol was activated. The patient was quickly examined clinically, given urgent laboratory tests, and had a CT scan of the brain to rule out hemorrhage.
The doctor diagnosed the patient with a left hemisphere cerebral infarction in the first hour, suspected of large vessel occlusion on a background of hypertension and atrial fibrillation.
The difficult problem is that the patient is taking anticoagulants, so doctors have to be very careful. The patient is given thrombolysis while undergoing contrast-enhanced CT angiography.
With the support of remote consultation from People's Hospital 115, the team decided to treat thrombolytic therapy according to the protocol, and at the same time had a CT scan of the brain to evaluate the cerebral blood vessels.
By 9:00 p.m. the same day (about 2 hours after the drug infusion), the patient's condition had improved significantly: muscle strength recovered 5/5, mouth distortion was gone, and language improved. Brain MRI showed left pontine infarction, and the left middle cerebral artery had completely reopened.
After more than 3 and a half hours since admission, the patient recovered almost completely, could do activities on his own, and his quality of life was guaranteed.
The Director of Hoc Mon Regional General Hospital said that this success is the result of a systematic investment process in the stroke unit. The unit currently has 5 specialized doctors, who are continuously trained and strictly follow the diagnostic and treatment procedures according to the standards of People's Hospital 115.
In the coming time, the hospital is committed to continuing to invest and develop the stroke unit in a specialized and sustainable manner, aiming at the goal of maximum recovery for patients.
