Another critical case of heart perforation was successfully treated thanks to the red alert procedure

That is the case of patient TVD (born in 1989) with address in Tran De town, Soc Trang province. The patient was admitted to the General Emergency Department at 8:05 a.m. on May 24, 2023 in a state of agitation, restlessness, rapid pulse, blood pressure dropped to 70/40mmHg, with 2 wounds in the precordial area about 2cm long each.

Immediately after admission, the doctors performed an echocardiogram and chest X-ray at the bedside. The results showed a large amount of left pleural effusion and a small amount of pericardial effusion; the heart was displaced to the right. The emergency room immediately thought that the patient was in shock and had lost blood due to a heart wound and immediately activated the internal red alert procedure.

After activating the procedure, the patient was urgently transferred to the operating room at 8:15 a.m. the same day. In the operating room, the team opened the chest and opened the pericardium. After 30 minutes of surgery, the patient's heart wound due to left ventricular perforation was sutured and the bleeding was well controlled.

By the morning of May 25, 2023, the patient was conscious, on a ventilator, with stable pulse and blood pressure, and no bleeding from the pleural and pericardial drainage. According to the doctors' assessment, the patient was out of danger. It is expected that in the next 24 hours, the patient will be transferred to the Department of Surgery for further post-operative monitoring.

Deputy Head of the General Emergency Department, Nguyen Dinh Chieu Hospital, Nguyen Khac Tri, said: This is a serious and critical case of heart perforation. If surgery is not performed promptly, the patient will certainly die. To successfully perform the surgery, it must be performed as quickly as possible by thoracic specialists to open the chest, open the pericardium, and promptly suture the heart wound to have a chance to save the patient's life.

Under the direct direction of the leaders of Nguyen Dinh Chieu Hospital through the internal red alert process with the smooth and urgent coordination of the departments: General Emergency, Surgical Anesthesia - Resuscitation, Anti-poison Resuscitation, Testing, the patient was successfully resuscitated.

The internal red alert procedure has been implemented by Nguyen Dinh Chieu Hospital since 2017. To date, 49/58 emergency cases have been successful thanks to this procedure. Since the beginning of 2023, 5/6 cases have been successfully performed, helping patients escape the jaws of death.

In the near future, Nguyen Dinh Chieu Hospital will develop an additional internal emergency red alert procedure for coronary heart disease to increase the chance of protecting the patient's life.

Compared to the normal process, a patient entering the emergency room must go through the following steps: medical examination, clinical tests, consultation, operating room preparation... and then surgery. With the red alert process, after receiving the order, medical staff from various departments will focus on coordinating and carrying out activities simultaneously, focusing on emergency treatment.

The advantage of the red alert procedure is that the medical staff mobilizes many departments, concentrates equipment and techniques to save the patient in an extremely short time, only 5 - 10 minutes, the patient is transferred directly from the emergency room to the operating room.
