The Central Endocrinology Hospital ( Hanoi ) recently admitted a 57-year-old female patient for treatment with fibrosis in the anterior cervical region. Previously, the patient had been treated for thyroid tumors by alcohol injection for a long time at a private facility.

The Central Endocrinology Hospital said this was a rare case. The patient had Basedow's disease with multiple nodules in both lobes of the thyroid gland and had been injected with alcohol for a long time at a private facility, causing fibrosis in the anterior neck area; paralysis of the left recurrent laryngeal nerve, left jugular vein obstruction, and difficulty breathing.
Doctors at the Central Endocrinology Hospital determined that the difficulty with this surgery was accessing the thyroid gland through a large fibrotic mass in the neck; removing the vascular thyroid gland with a high risk of bleeding due to Graves' disease, while preserving the carotid artery bundle that had been encircled by fibrotic tissue.
The surgical team had prepared a plan for a tracheostomy to maintain the airway if there was damage to the remaining recurrent laryngeal nerve.
The patient was treated with techniques to constrict the thyroid blood vessels, limit bleeding, and regulate hormones to normal thyroid levels. After surgery, all thyroid tissue and fibrosis were removed, the laryngeal nerve related to the voice was exposed and preserved, and the patient remained stable.
From the above case, the Central Endocrinology Hospital recommends that when having thyroid disease, patients need to consider when choosing and approaching treatment methods; need to examine and treat thyroid disease at specialized facilities.
For medical staff, when treating patients, it is necessary to comply with the indications for alcohol injection for appropriate cases, coordinate with ultrasound throughout the treatment process to ensure correct technique, accuracy, avoid abusing alcohol injection treatment for a long time; change appropriate treatment methods such as surgery for patients with Basedow's disease.
