The ejaculatory area has two muscles: the external urethral sphincter and the internal urethral sphincter at the bladder neck.

During normal ejaculation, the internal sphincter closes, the external sphincter opens, and semen flows from inside to outside.

Retrograde ejaculation is when a man ejaculates, but the semen goes back into the bladder because the internal sphincter is relaxed. The cause is usually an opening in the bladder neck.

Bladder neck incompetence occurs in cases of:

- After surgery that affects the closure of the bladder neck such as bladder neck incision, after endoscopic prostatectomy...

- Bladder neck opening due to diseases such as diabetes, after retroperitoneal lymph node dissection...

In the case of retrograde ejaculation, during intercourse, semen does not enter the woman's vagina, resulting in the couple being unable to have children.
