Dr. Vo Trieu Dat - Head of Obstetrics and Gynecology Department of FV Hospital said this is a rare case where a patient with hemorrhage refused to have a hysterectomy - while this is a mandatory treatment indication . Due to respect for the patient's rights, doctors had to spend many hours just explaining and convincing the patient to accept the treatment.
Doctor Vo Trieu Dat and the Obstetrics and Gynecology Department of FV Hospital performed emergency surgery on a Turkish pregnant woman.
Persistently convince to treat the patient
Ms. J. S. (40 years old) has given birth to 3 children (1 time giving birth to twins) and has a history of cesarean section . During her fourth pregnancy, she went to FV Hospital examined the pregnant woman at 33 weeks of pregnancy. Doctor Vo Trieu Dat said that all of the pregnant woman's medical records from a previous examination in another country had been lost.
On January 3 , 2025 , Ms. JS was admitted to the hospital at 3:30 p.m. and 30 minutes later gave birth to a healthy baby boy weighing 3.4 kg. However, the mother later experienced vaginal bleeding . Despite the balloon insertion procedure , the bleeding continued, indicating postpartum hemorrhage. At this time, the doctors decided to take the mother to the operating room to stop the bleeding, not excluding the possibility of hysterectomy .
"Postpartum hemorrhage is a dangerous obstetric complication that can occur in women without risk factors. In Ms. JS's case, postpartum hemorrhage occurred due to placenta accreta near the old surgical wound. This is an extremely dangerous situation . If not treated promptly, the mother may lose blood to the point of death ," said Dr. Vo Trieu Dat .
Placenta accreta can lead to complications of postpartum hemorrhage.
However, the mother and her husband opposed the surgery, they just wanted to stay in the delivery room and monitor the progress. Meanwhile , the entire surgical team including anesthesiologists and obstetricians were on standby, ready for surgery at any time.
"From 7:30 p.m. until 1 a.m. the next morning, there was a mental struggle between the medical team and the patient and her husband. We were very worried about the life of the pregnant woman and tried to convince the patient to agree to treatment. Although they understood the seriousness of the problem, they still calmly requested to continue the blood transfusion and monitoring," Dr. Dat recalled the tense struggle. There were times when the doctors could not keep calm and were helpless, especially when the pregnant woman showed signs of hemorrhagic shock, low blood pressure, and rapid pulse. The patient was sometimes in a state of confusion due to hemorrhagic shock, but when she was awake, she still resolutely refused surgery, while the husband still respected his wife's opinion. The doctors were confused and worried. when standing between the desire to save lives and the patient's right to decide on treatment.
A solution was proposed by the doctor to perform an embolization procedure to stop the bleeding for the pregnant woman, but 30 minutes later the bleeding resumed. At this point, the pregnant woman's blood pressure dropped sharply and the patient fell into a very critical state of shock.
When life comes first
Dr. Dat realized that the patient's condition was deteriorating very quickly. Although he respected the patient's decision, if he did not treat her immediately, it would be difficult to save the pregnant woman's life. " We had to take the pregnant woman into the operating room immediately. At that time , we had to put the patient's life first, even if they refused treatment , " Dr. Dat recounted.
After 5 hours of persuasion, the pregnant woman was taken to the operating table.

The pregnant woman was rushed into the operating room, and Dr. Dat discussed with the patient's husband once again: there was no other intervention other than surgery to remove the uterus to stop the bleeding. At this point, realizing the seriousness of the situation and not being able to delay, the husband agreed to let the doctors remove his wife's uterus to stop the bleeding.
The surgery took place at the time 1:00 a.m. Dr. Dat successfully removed the patient's uterus and stopped the bleeding. In total, the patient lost 2.5 liters of blood, fortunately she recovered quickly. fast
Discharged from the hospital 3 days later, JS and her husband expressed their gratitude to the FV team . They both expressed that they truly understood the seriousness of the situation and knew that the doctors had done everything to save her life .
