Ho Chi Minh City: A baby girl was born with a distended belly due to a giant hemangioma in the liver - only 5 cases recorded in world literature. She was transferred from Ca Mau to Children's Hospital 1 for surgery.

On October 12, Dr. Dang Nguyen Quynh Nhu, Department of General Surgery, Children's Hospital 1, said that the baby girl was admitted to the hospital at 2 days old and had a hemangioma detected at 21 weeks of pregnancy. Imaging tests showed a hemangioma in the left liver.

The child's blood clotting disorder was getting worse and worse, and his platelet count was decreasing. Doctors consulted with the entire hospital and decided that surgery was the top priority, despite the danger.

Dr. Dao Trung Hieu, a consultant in the surgical department, said that if the surgery was delayed, the baby would have heart failure because of the giant hemangioma with connections between the arteries and veins. At that time, the surgery would be difficult to succeed. Before the surgery, the doctors prepared many plans, determined the location of the skin incision, and prepared the tools to safely cut the liver.

The baby was given a large amount of blood and blood products to be ready for surgery at 9 days old. The liver of a newborn baby is very soft and friable, making it difficult to stop bleeding. The patient also had venous connections in the liver, so the surgery was very difficult and required careful dissection to minimize the risk of bleeding.

After 112 minutes, the surgical team of 10 doctors successfully removed the left liver containing the hemangioma, the tumor size was about 7x8cm, occupying more than 1/5 of the baby's body. After the surgery, the baby was on a ventilator for two days, recovered well and was discharged after 11 days of treatment.

This is the fourth case of liver hemangioma in a newborn to be operated on at Children's Hospital 1. The previous cases had smaller tumors and more preparation time. This case had the largest liver hemangioma and was performed most urgently, without time for endovascular intervention like the previous cases.

Hemangioma of the liver is very rare, the cause is still unclear. Doctors recommend that pregnant women have regular check-ups, screening for abnormalities to have timely intervention, and treatment plans for both mother and baby.
