Multiple collisions during traffic jam leave pregnant woman in critical condition
On the morning of July 19, Ms. NTT, 38 years old, Ninh Binh, and her husband left home to go to the hospital for a routine pregnancy check-up at week 37. On the way, when they reached a traffic jam, Ms. T. and her husband's car was stopped when suddenly a truck behind them could not handle the situation and hit the back of the car. The impact was so strong that Ms. T.'s car was pushed up and hit the back of the truck in front, the front of the car was under the truck, causing all three cars to be severely damaged.
“My car was crushed in both the front and the back, and the truck in front and behind was also dented. Immediately after the collision, I felt a lot of pain in my lower abdomen and was bleeding. Luckily, my husband and I were still conscious at that time,” Ms. T. recounted.
Faced with the urgent situation, Ms. T's husband immediately got out of the car and called for help from passersby. A police car passed by and helped them get to Thu Cuc TCI International General Hospital as quickly as possible.
Warning signs from the first minute of reception
At the Department of Obstetrics - Thu Cuc TCI International General Hospital, Ms. T. was admitted urgently with abnormal signs: lower abdominal pain, vaginal discharge of bright red blood, average physical condition, pale mucous membranes.

Obstetricians and Gynecologists are urgently performing emergency surgery to save the lives of mother and child.
Ultrasound results showed the fetus was in the head position, the fetal heart rate was only 69 beats/minute (normal 120-160 beats/minute), the rhythm was irregular and sporadic, showing signs of fetal distress. The fetus was estimated to weigh 2,800 grams, the pelvic image showed free, echogenic fluid next to the uterus. The vagina was bleeding bright red, about 200 ml.
The patient had a history of 2 cesarean sections. Combining clinical examination and ultrasound, the patient was consulted and diagnosed with: 37 weeks pregnant, 2 previous cesarean sections, uterine rupture due to traffic accident, severe fetal distress. The patient was assigned to be transferred to the operating room for emergency surgery.
The major surgery was stressful, fighting for the lives of mother and child every second.
When opening the abdomen, the uterus was found to have ruptured across the lower part of the old surgical scar, tearing down to the left uterine isthmus. There was more than 300 ml of red blood and blood clots in the abdomen. A baby boy weighing 2,800 grams was taken out, suffering from severe asphyxia. After intensive resuscitation, after 5 minutes the baby was able to cry, after 10 minutes, the APGAR score was 10 points. The baby was transferred to the Neonatal Unit's intensive care unit for further treatment.
Doctors noted that the uterus had ruptured at the site of the old surgical scar, the tear extending deep down to the uterine isthmus near the internal opening of the cervix. The entire lower segment of the front and back of the uterus was hematoma - a complex injury, the uterus could not be preserved, so a low partial hysterectomy was ordered, leaving two appendages.
Because it was a case of uterine rupture due to a traffic accident, it could damage solid organs such as the liver, kidneys, spleen and hollow organs such as the stomach, colon, small intestine as well as the ribs, limbs and brain. Therefore, after treating the obstetric injuries, the obstetricians invited a consultation with surgeons to coordinate the examination of those organs to see if there were any injuries. Fortunately, after the examination, no injuries were detected.
During the surgery, the patient was transfused with 4 units of blood and clotting factors. After more than 2 hours and 30 minutes, the surgery was a success. The patient was transferred to the intensive care unit of the Department of Anesthesia and Resuscitation for further monitoring and treatment.

After the baby was safely delivered, doctors from the Obstetrics and Gynecology Department and the Surgery Department worked together to handle the mother's problems.
Dr. Nguyen Van Ha - Deputy Director, Head of Obstetrics and Gynecology Department, Thu Cuc TCI International General Hospital - who directly examined and performed the emergency surgery said: "When receiving the pregnant woman, the prognosis for the mother and fetus was very dangerous. When we operated, the pregnant woman's uterus had ruptured and was bleeding a lot. Fortunately, we were able to save the baby. The ruptured uterus was very complicated, so the uterus could not be preserved, so we had to partially remove the uterus, leaving 2 appendages.
After 7 days of treatment, the health of the mother T. has stabilized, the surgical wound is dry, no fever, and good spirit. The baby boy has returned to bed with his mother, eats and sleeps well, and his vital signs are normal. The patient was discharged from the hospital to the joy of his family as well as the doctors of the Department of Obstetrics and Gynecology, Department of Surgery, and Department of Anesthesia and Resuscitation - Thu Cuc TCI International General Hospital.
Dr. Nguyen Van Ha added that this is a great success and the close coordination between the Department of Obstetrics and Gynecology, Department of Surgery, Department of Anesthesia and Resuscitation, and Department of Testing.

The baby was born at 37 weeks, received special care and is now in stable health.
Recalling the recent life-and-death accident, the mother T emotionally said: “When the baby was brought out, it was in a serious condition because the mother had a ruptured uterus, so she had to receive intensive care and treatment from the Neonatal Unit. The couple and family were extremely worried about the baby’s condition. However, thanks to the intensive and enthusiastic treatment and care from the doctors and nurses, today the baby’s health has returned to normal and she has returned to her mother.
My health is stable today and I was discharged from the hospital today. The mother's family is happy because both mother and baby are safe.
The husband of the pregnant woman could not hide his emotion when recalling his wife's life-and-death journey: "If we had been a little slower, we would not have been able to save mother and child.
