Activate 'red alert' to save female student in critical condition after traffic accident

DNVN - On the afternoon of October 10, Da Nang Hospital announced that the hospital's doctors had just activated the "Internal Red Alert" procedure, coordinating with other specialties to save the life of an 18-year-old female student at risk of death due to a traffic accident.

Previously, patient TNPU (18 years old, residing in Ba Na commune, Da Nang city) was transferred by the 115 Emergency Center to Da Nang Hospital in a state of multiple trauma, severe blood loss, respiratory failure, and circulatory failure due to a traffic accident. The patient was lethargic, had pale skin and mucous membranes, a distended abdomen, chest and abdominal pain, a deformed left arm, unmeasurable blood pressure, and an undetectable peripheral pulse. An urgent ultrasound detected a lot of blood in the abdomen.

Emergency surgery at Da Nang Hospital saves the life of a female TNPU student

Recognizing this as a top emergency, the hospital activated the “Internal Red Alert” procedure. Accordingly, all resources were mobilized and the patient was taken straight to the operating room with a diagnosis of multiple injuries, closed abdominal trauma, closed chest trauma, left arm fracture, and head and face trauma.

Within minutes, doctors and nurses from many related specialties such as Anesthesia, Gastroenterology, Thoracic Surgery, Neurosurgery, Urology, and Trauma Surgery were present in the operating room, coordinating anesthesia, resuscitation, and surgery in parallel.

The teams performed surgery to simultaneously drain the pleura, stop bleeding in the left arm, and perform abdominal surgery. Dr. Tran Van Nghia (Digestive Surgery Department, Da Nang Hospital) said that when opening the abdomen, a series of severely damaged organs were discovered: grade IV spleen rupture, grade II liver injury, grade I left kidney injury, stomach rupture, small intestine rupture, colon rupture, and blood in the abdomen.

Doctors prioritize hemostasis for patients by performing splenectomy and parallel liver suturing, wedge gastrectomy, small intestine resection, and colon resection with Stapler to quickly stop bleeding and shorten surgery time.

The surgery lasted nearly 5 hours under conditions of continuous resuscitation and surgery to fight for every minute of life for the patient.

“This is an extremely complicated case, life is only measured in minutes. If there is a delay, the patient may not survive,” said Dr. Tran Van Nghia.

Mr. Nghia shared that during the surgery, the patient had to be transfused with 10 units of blood and blood products, and used high doses of vasopressors to maintain hemodynamics. The anesthesia and resuscitation teams and the surgical team coordinated quickly, promptly, and accurately, so they were able to handle the injuries well and stabilize hemodynamics right on the operating table.

According to Dr. Ha Phuoc Hoang (Deputy Head of Anesthesia and Resuscitation Department, Da Nang Hospital), the key factor in saving the patient's life is the timely activation of the "Red Alert" procedure, along with the smooth coordination between specialties, the extensive experience of the surgical team, anesthesia and resuscitation, and the use of modern hemostasis techniques.

After surgery, the patient was put on a ventilator and actively resuscitated in the Surgical Intensive Care Unit (Department of Anesthesia and Resuscitation). After 10 days of treatment, the patient's health condition improved and he was transferred to the Department of Gastroenterology for further monitoring. Currently, the patient is awake, eating normally, has stable vital signs and has been discharged from the hospital.
