Successful surgery to remove "giant" thyroid tumor for female patient

On the afternoon of October 4, a representative of the leadership of Da Nang Hospital said that doctors from the Department of Thoracic Surgery, Da Nang Hospital had just successfully performed surgery to remove the entire thyroid goiter measuring 15x10x10cm for each lobe of a 24-year-old female patient.

Patient Ha TP (born in 2000, residing in Gio Linh district, Quang Tri province) has had Basedow's disease for 12 years. In the past 3 years, since becoming pregnant and giving birth, the patient's eyes have become increasingly bulging, and the goiter in her neck has grown larger. The patient has gone to many places for treatment, but because the goiter is large and has many other diseases, the doctor only prescribed medication and did not perform surgery to remove the goiter.

The patient was admitted to Da Nang hospital for emergency treatment with a high fever. After being diagnosed and treated for fever, the patient was transferred to the Thoracic Surgery Department for further treatment and tumor removal surgery.

According to specialist II Doctor Than Trong Vu, Head of the Department of Thoracic Surgery, Da Nang Hospital, the patient was admitted to the Department of Thoracic Surgery with bulging eyes, a large tumor causing difficulty breathing, difficulty swallowing, and low white blood cells due to antithyroid drugs. The patient was indicated for surgery.

However, the patient's hyperthyroidism is not stable, the thyroid gland is too large, which will cause difficulty in anesthesia, the risk of bleeding during and after surgery is very high. At the same time, there will be a risk of hoarseness, loss of voice and hypocalcemia after surgery due to damage to the nerves that control the voice and parathyroid glands as well as the risk of difficulty breathing after anesthesia due to the trachea collapsing due to long-term compression of the large goiter, leading to tracheomalacia.

Thoracic surgeons decided to increase the dose of antithyroid drugs combined with lugol's solution, beta blockers and sedatives, and inject prophylactic antibiotics to stabilize the patient.

After nearly 2 weeks of active treatment, the patient was scheduled for surgery to remove the entire thyroid gland.

The surgery was very difficult because the thyroid gland was very large, retreating behind the trachea and hanging down into the chest. There were many hyperplastic blood vessels that easily caused bleeding when touching the tumor. The large tumor pushed the nerve structures that controlled the voice and parathyroid glands, causing narrowing of the trachea.

However, after nearly 4 hours of surgery, doctors from the Thoracic Surgery and Anesthesia and Resuscitation departments successfully removed the patient's entire thyroid gland without any complications.

Currently, the patient no longer has difficulty breathing, can eat and talk normally, and has no muscle spasms in his arms and legs.

Doctor Vu added that Basedow is an autoimmune disease, the immune system attacks and stimulates the thyroid gland to produce excessive hormones, leading to thyroid hormone poisoning. As a result, the patient has symptoms in many organs such as: bulging eyes, nervousness, irritability, rapid pulse, large goiter, myxedema...
