7 year old patient with complex femur fracture

Hanoi Children's Hospital received patient D.HP, 7 years old, from Chuong My, Hanoi, transferred from a lower level hospital with a condition of immobility of the left leg and sharp pain in the thigh.


After being examined and X-rayed by doctors, the results showed that the patient had a femoral shaft fracture, a closed fracture of the middle third of the left femur, and was indicated for bone fusion surgery using intramedullary nailing technique on a C-arm irradiation monitor.

The surgical team led by Dr. Nguyen Dang Bang and Dr. Pham Hoang Anh, Department of General Surgery; Anesthesiologist Giang Thach Thao and technician Phung Xuan Thao performed closed bone fusion surgery using intramedullary nails. This is a minimally invasive and effective surgical method that helps patients move and recover quickly. After just over an hour, the surgery was a great success. The broken bone was anatomically adjusted and firmly fixed without opening the fracture site.

Previously, pediatric patients with fractures requiring surgery were often operated on with screws and plates, a technique that required exposing the fracture site, causing damage to soft tissue as well as the periosteum and blood vessels that nourish the bone. This technique had disadvantages such as increased risk of infection, delayed bone healing, joint stiffness due to prolonged immobilization, and unsightly scars.

Orthopedic specialists, Department of General Surgery of Hanoi Children's Hospital have implemented a bone fixation treatment method using intramedullary nails with many advantages such as: Minimally invasive, no fracture opening; shortened bone healing time thanks to maximum preservation of periosteum and blood vessels; small surgical scar, high aesthetics; short hospital stay, only need to use a single dose of prophylactic antibiotics before surgery.

After surgery, the patient's condition is stable, the surgical scar is very small, ensuring high aesthetics, and he is expected to be discharged after 2 days of surgery. Doctors advise the patient to exercise early to avoid stiff knees and to lean on his legs when conditions permit.

According to doctors, the surgical technique of combining bone fusion on a light-increasing screen to treat all types of fractures in general and femur fractures in particular is a modern treatment method, applying advances in science and technology. Children treated with this method can recover early, quickly return to daily activities, helping children minimize fear and obsession in the future.
