Difficult and challenging heart transplant to save end-stage heart failure patient

According to doctors, this is a difficult and challenging transplant to save a patient with dilated cardiomyopathy and end-stage heart failure.

Previously, on March 2, upon receiving information from the National Organ Transplant Coordination Center that there was an organ donor in Ho Chi Minh City, Prof. Dr. Pham Nhu Hiep, Director of Hue Central Hospital immediately activated the organ coordination team. Doctors from the hospital were assigned to coordinate with Gia Dinh People's Hospital and the National Organ Transplant Coordination Center to receive the organ.

After calculating the time to retrieve the heart at Gia Dinh People's Hospital and the time to prepare the patient for the transplant at Hue Central Hospital, thanks to the support and professional and smooth coordination between the teams, flight number VN1374 brought the heart to Hue at 18:54 on March 3.

According to doctors, this is a difficult and challenging transplant. The patient has dilated cardiomyopathy, end-stage heart failure, severe pulmonary hypertension, and poor response to medical treatment; the risk of heart failure and pulmonary hypertension after heart transplant surgery is very high, requiring preventive treatment options after the transplant.

In addition, the donor's heart also faced many challenges: long recovery time, high doses of vasopressors were used, and the quality of the heart was not guaranteed according to the first test.

After the third brain death consultation, re-evaluation and adjustments based on expert opinions, such as reducing the use of vasopressors and close monitoring, the re-evaluation found that the quality of the donor's heart had improved and improved.

After the heart transplant, the patient ate and exercised lightly on the recovery bed. Blood and biochemical tests were within normal limits.

After 3 hours and 48 minutes from the time the heart was received and transported to Hue Central Hospital, the donor's heart started beating again in the chest of the patient, Mr. NVC, 36 years old, residing in Quang Nam , at 8:38 p.m. on March 3.

The patient had severe heart failure and hypertension after the transplant, so weaning from extracorporeal circulation was very difficult. The resuscitation team used inotropic and pulmonary vasodilator drugs, along with the extracorporeal circulatory and respiratory support system (ECMO) to successfully wean from extracorporeal circulation.

After surgery, the patient was transferred to the recovery room for continued intensive resuscitation, mechanical ventilation, use of drugs to treat heart failure and pulmonary hypertension, and maintenance of ECMO.

After 3 days of ECMO support, the patient's hemodynamics were stable, pulmonary artery pressure was well controlled, and he was weaned off ECMO and ventilator. The patient was eating and exercising lightly on the recovery bed, and his hematological and biochemical tests were within normal limits.
