Timely rescue of mother and child from placental abruption

Can Tho University Hospital successfully performed emergency surgery on a pregnant woman with premature placental abruption. After surgery, both mother and child recovered well.

Placental abruption is an obstetric emergency, a serious systemic disease that rapidly progresses and causes death of both mother and child. The cause is that the placenta is properly attached but separates before the fetus is born due to the formation of a blood clot behind the placenta. The blood clot gradually grows and separates the placenta and membrane from the uterine wall, cutting off the oxygen exchange between mother and child.

The incidence is 0.6% - 1%. Placental abruption is the cause of death in 10 - 15% of stillbirths in the last 3 months.

Risk factors such as:

- Preeclampsia

- Sudden excessive stretching of the uterus

- Sudden change in uterine volume such as amniocentesis in polyhydramnios, or twin pregnancy after the first fetus is delivered

- Short umbilical cord

- Trauma

- Folic acid deficiency, often found in pregnant women with low living standards

- Smoking, using Cocaine

Placental hematoma: formed from maternal blood, can be small or large from 500 - 1500g, this hematoma gradually detaches the placenta, if the hematoma is small, the placenta detaches slightly, the fetus can live. If the hematoma is large, the placenta detaches completely, the fetus dies. The hematoma will separate the uterine muscle fibers, fluid and blood will seep into the abdominal cavity, the uterine muscle will lose its elasticity and contractility, so after giving birth, it is easy to get uterine atony if it does not recover, in some cases, the placenta must be cut immediately to stop the bleeding.

Placental abruption (blood clot behind the placenta)

Pregnant woman Ta Thi Xuan M (1989, Can Tho), first pregnancy, 36 weeks gestation; after a business trip by motorbike of more than 150km, the pregnant woman had abdominal pain so she went to the hospital for examination. However, on the way, the abdominal pain got worse, after examination after admission (clinical examination, ultrasound, monitoring measurement...) with the diagnosis of moderate placental abruption and an emergency cesarean section was performed immediately, the baby girl weighed 2600 grams

The successful surgery was performed by Dr. Nguyen Thi Hue - Head of Obstetrics and Gynecology Department and Dr. Tran Thanh Dong - Obstetrics and Gynecology Department, Nam Can Tho University Hospital. The mother was kept in the Obstetrics and Gynecology Department for monitoring and special care. The baby was given respiratory support after birth and was able to breathe normally after resuscitation.
