HSSKĐT is an important tool that helps people monitor their entire health care process throughout their lives - from birth to old age, from vaccination data, school check-ups, periodic check-ups to inpatient and outpatient treatment information. With the integration on the Ho Chi Minh City Digital Citizen app, it will help people easily look up and monitor their personal and family health status with just a mobile device.
Recognizing the importance of creating health data for city residents, most clearly creating health data for residents during the recent COVID pandemic, the City Department of Health has built and completed the "Community Health Management" platform.

With this platform, from 2023, the Department of Health will use it to manage the health of the elderly. Next, from 2024 onwards, in addition to the health data of the elderly, the Department of Health will coordinate with the Departmentof Education and Training to deploy the management of periodic health examination data of students throughout the city.
Since April 2025, the health sector has officially started creating an integrated electronic health record on the Digital Citizen app, starting with integrating the electronic health record of the elderly. Up to now, 396,794 electronic health records of the elderly have been successfully synchronized.
When the city's medical data warehouse is completed and connected, the people's electronic health care system will continue to be expanded, integrating more data from medical examination and treatment facilities, helping to manage and treat more effectively, contributing to the realization of comprehensive health care for people in the digital transformation era.
