
According to new regulations, the Ministry of Health clearly states that people with one of the following medical conditions or disabilities are not qualified to drive railway vehicles, including:
For psychiatric specialties: Mental illness has been completely stabilized but not for 24 months.
For neurology: Epilepsy; paralysis of one or more limbs; extrapyramidal syndrome; superficial sensory disturbances or deep sensory disturbances; dizziness due to pathological causes.
For ophthalmology: Distant vision of each eye: good eye <8/10 or poor eye <5/10 (including correction with glasses); refractive error with glasses number: > +5 diopters or > -8 diopters; disorder in recognizing 3 basic colors: red, yellow, green; double vision; glare diseases; reduced vision at dusk (night blindness)...
For ENT specialists: People with better hearing in the better ear when speaking normally < 4m (including the use of hearing aids); Or hearing minimal whispers (in the better ear) < 0.4m (including the use of hearing aids).
For cardiovascular specialties, the diseases are: Vasculitis (arteriovenous), vascular malformations with clinical manifestations affecting the ability to operate and control railway vehicles; arrhythmias: supraventricular tachycardia, ventricular tachycardia, atrial flutter, atrial fibrillation, atrial tachycardia and sinus tachycardia > 120 cycles/minute, treated but not yet stable.
Ventricular extrasystole in people with structural heart disease and/or grade III or higher according to Lown's classification; second-degree atrioventricular block or bradycardia accompanied by clinical symptoms (including those treated but not stable); angina due to coronary artery disease; heart transplant; after coronary revascularization intervention; heart failure grade II or higher (according to the classification of the New York Heart Association - NYHA)...
For respiratory specialties, these are diseases and disabilities that cause difficulty breathing at level II or higher (according to mMRC classification); partially controlled or uncontrolled bronchial asthma; and infectious tuberculosis.
For musculoskeletal specialties: Stiffness/adhesion of a large joint; pseudo-joint at a position belonging to (in) large bones; excessive kyphosis, scoliosis causing lordosis; stiffness/adhesion of the spine affecting motor function; absolute length difference between two upper or two lower limbs of 5cm or more without assistive devices; amputation or loss of function of two or more fingers of one hand or amputation or loss of function of one or more feet...
For endocrinology, it is the case of a person with diabetes (diabetes) with a history of diabetic coma within 1 month.
Along with that, the Ministry of Health also stipulates that the use of drugs, alcohol, narcotics and psychotropic substances is also in the group of people who are not qualified to drive railway vehicles, including: Using drugs; using substances with alcohol concentration (applicable during periodic health check-ups); using treatment drugs that affect the ability to wake up; abusing stimulants and hallucinogens.
The Circular of the Ministry of Health also clearly stipulates the responsibility of health examination facilities to comply with the provisions of the law on health examination of the Law on Medical Examination and Treatment and documents guiding the implementation of the Law on Medical Examination and Treatment.
At the same time, issue forms and issue health certificates that ensure they fully contain the minimum information specified in Appendix 02 and Appendix 03 issued with this Circular and fully update health examination information as prescribed in Article 5 of Circular No. 36/2024/TT-BYT dated November 16, 2024 of the Minister of Health regulating health standards, health examinations for drivers and specialized motorbike operators; periodic health examinations for car drivers; database on health of drivers and specialized motorbike operators.
Regarding transitional provisions, the Circular clearly states: In case the health examination has been performed before the effective date of this Circular, but the conclusion has not been issued, the conclusion shall be implemented according to the provisions of Circular No. 12/2018/TT-BYT of the Minister of Health regulating the health standards of railway staff directly serving train operations.
Circular No. 42/2025/TT-BYT takes effect from January 1, 2026.
