To buy health insurance online for your family, you need to determine whether you belong to this type of insurance group and consult the contribution level before deciding.
Who can participate in family health insurance?
According to Article 5 of Decree 146/2018, amended and supplemented in Decree 104/2022, the group participating in family health insurance includes:
1. People whose names are in the same household registered for permanent residence, except for those subject to the provisions of Articles 1, 2, 3, 4 and 6 of Decree 146/2018 including:
(1.1) Group contributed by employees and employers
(1.2) Group paid by social insurance agency
(1.3) Group funded by the state budget
(1.4) Groups whose contribution levels are supported by the state budget
(1.5) Employer-paid group
2. People whose names are in the same household registered for temporary residence, except for subjects (1.1), (1.2), (1.3), (1.4), (1.5) and subjects who have participated in health insurance as prescribed in (1).
3. The following subjects are eligible to participate in family health insurance:
- Dignitaries, officials, monks;
- People living in social protection facilities except for subjects (1.1), (1.2), (1.3), (1.4), (1.5) mentioned above who are not supported by the state budget to pay health insurance.

Family health insurance premiums
The first person pays 4.5% of the basic salary; the second, third, and fourth people pay 70%, 60%, and 50% of the first person's contribution, respectively; from the fifth person onwards, the contribution is 40% of the first person's contribution.
The current basic salary is 2,340,000 VND/month. Thus, the family health insurance contribution is as follows:
- The first person is 105,300 VND/month.
- The second person is 73,710 VND/month.
- Third person is 63,180 VND/month.
- The fourth person is 52,650 VND/month.
- From the fifth person onwards is 42,120 VND/month.
Note: Participating in family health insurance does not require the whole family to pay at the same time, but can buy multiple times during the financial year (January 1 to December 31 of the year) and still receive the above deductions. Participants can choose to pay periodically every 3 months, 6 months or 12 months.
Steps to pay family health insurance online
Step 1 : Log in to the National Public Service Portal (https://dichvucong.gov.vn)

Step 2 : After successfully logging in, on the menu bar, select "Online payment" and then click on "Pay voluntary social insurance and health insurance by household"

Step 3 : On the screen, in the section "Renewing health insurance card for household", you choose "Reduced contribution level"

The system will then switch to the public service portal of Vietnam Social Security. You fill in all the information of the members of the household participating in Health Insurance in the same declaration and are responsible for the content of the declaration.

Step 3: The software system automatically checks and compares the information of the declared participant.
The software will determine the place of permanent or temporary residence registration, verify the information of the head of household and household members who are registered for permanent or temporary residence with the National Population Database, determine the order of health insurance participation of each member to be deducted from the contribution level in the Vietnam Social Security database and determine the exact amount to be paid for participants to pay online.
In case the residence is determined or the information of the household head is not correctly paid or the members are not registered for permanent or temporary residence with the National Population Database, the software system will display a notification to inform the participant about the residential address to contact the police agency for instructions and adjustments, then repeat Step 2 or can register to participate in health insurance at social insurance and health insurance collection service organizations or at the social insurance agency.
Step 4 : Participants make online payments on the Public Service Portal, receive electronic receipts, and receive notifications of the health insurance card payment deadline or the health insurance card's continued validity period immediately after the transaction is successful.
Step 5: Participants receive electronic health insurance cards or paper health insurance cards according to the registered method.
*Note : After successfully registering, if you want to renew your health insurance card, in step 3, select "no deduction of payment", the system will display an interface for you to enter your health insurance card code as below. You choose the number of months you want to renew, check the information and pay via the linked bank.
