Emergency surgery for 2 male students after severe pain in private area

Recently, the Department of Nephrology and Urology, Bac Giang General Hospital, received two cases of young men hospitalized with late testicular torsion, forcing one testicle to be removed due to necrosis.

The first case is HTA (16 years old, residing in My Thai commune, Lang Giang district). According to the family, about 4 days before being admitted to the hospital, he suddenly had severe pain in the left scrotum.

At first, I thought it was just a normal pain, so I subjectively did not go to the doctor. After a day, the pain did not subside, my scrotum began to swell, but I was still afraid and did not tell my family. It was not until the pain became severe and accompanied by vomiting that I was taken to the hospital.

Doctors diagnosed the patient with left testicular torsion and ordered emergency surgery, but because the testicle was black and necrotic and could no longer be preserved, it had to be removed to avoid complications of infection and affect the remaining testicle.

The second case is TQT (18 years old, residing in Thanh Lam commune, Luc Nam district). Similarly, he had pain in his right scrotum 6 days before being admitted to the hospital. At first, the pain appeared suddenly, but he thought it was due to vigorous exercise so he did not pay attention.

When I saw my scrotum swell, the pain became more severe and it was red, I still hesitated to go to the doctor because I was afraid of the sensitive issue. Only when the pain became unbearable did I go to the hospital.

Doctors diagnosed him with right testicular torsion and ordered emergency surgery. However, the testicle was severely necrotic and could not be saved, so doctors had to surgically remove the right testicle.

After surgery, both patients recovered well and can be discharged in the near future. However, losing one testicle can affect the psychology, fertility and physiological health of the patient later.

What is testicular torsion and why should it be treated early?

According to doctors, testicular torsion (also known as spermatic cord torsion) is a condition in which the testicles rotate abnormally around the spermatic cord, causing the blood vessels that supply the testicles to become blocked. If not treated promptly, the testicles will be deprived of blood supply, leading to necrosis and having to be removed.

The disease is common in children and adolescents. Typical symptoms are sudden, severe scrotal pain, the pain can spread to the abdomen or down the thighs, accompanied by sweating.

The scrotum is swollen, red, painful, and the testicles feel pulled higher than normal. Some cases may be accompanied by nausea or vomiting. In addition, the patient may not have a fever or only a mild fever.

Testicular torsion is a surgical emergency. The golden time to save the testicle is within 4-6 hours of the onset of symptoms. If surgery is performed to untwist the testicle early, it can be saved. However, if hospitalized after 24 hours, the risk of necrosis and amputation is up to 85%.

One of the reasons why many patients come to the hospital late is because of subjectivity or embarrassment when encountering problems related to the genitals. This has led to the unfortunate consequence of losing one testicle, seriously affecting reproductive health and quality of life later on.

Therefore, doctors recommend that early detection and treatment is the key to preserving the testicles and protecting the patient's reproductive health. When symptoms such as severe scrotal pain, swelling, redness or other unusual signs appear, the patient should quickly go to a medical facility for timely emergency care.
