Da Nang: Successful surgery on rare isolated lung disease

After 2 hours, doctors at Da Nang Hospital successfully removed the isolated lung mass along with part of the parenchyma of the left lung lobe, preserving as much of the remaining lung parenchyma of the lower lobe as possible instead of removing an entire lobe.

On April 14, Da Nang Hospital announced that doctors from the Thoracic Surgery Department of the Hospital had successfully performed endoscopic surgery to remove an isolated lung mass for a 57-year-old male patient.

This is a rare congenital disease, rarely diagnosed in adults. Patient Nguyen TK (57 years old, residing in Hoa Tho Dong ward, Cam Le district, Da Nang city) discovered a lesion in the lower lobe of the left lung by chance during a general health check-up. 20 years ago, the patient had pneumonia, which was treated well, and now often has tingling pain in the left back area.

At Da Nang Hospital, the patient's chest CT scan showing a lesion in the lower lobe of the left lung, measuring 32x45mm, with blood vessels feeding directly from the thoracic aorta, about 11mm in diameter, and venous blood draining directly into the left lower pulmonary vein. The patient was diagnosed by the doctor with a separate lung mass in the lower lobe of the left lung.

Doctors from the Department of Thoracic Surgery performed a surgical procedure to remove the isolated lung using a two-port thoracoscopic technique, which helped minimize chest wall trauma for the patient. After 2 hours, the doctors successfully removed the isolated lung mass along with a portion of the left lung lobe parenchyma, preserving as much of the remaining lung parenchyma of the lower lobe as possible instead of removing an entire lobe.

Directly performing surgery on the patient, Specialist II Doctor Than Trong Vu, Head of the Department of Thoracic Surgery, Da Nang Hospital, said that this patient's case was quite complicated because the pulmonary artery was isolated and large. The patient was elderly, at risk of brittle artery walls, increased risk of bleeding, requiring absolute caution during surgery. Currently, the patient is stable and ready to be discharged.

According to Dr. Than Trong Vu, pulmonary sequestration is a rare congenital disease, accounting for 0.1% of congenital lung lesions. The disease is diagnosed in infancy. In adults, the diagnosis of pulmonary sequestration is rarely recorded. The progression of pulmonary sequestration can lead to recurrent pneumonia, lung abscess, cancer, severe bleeding, life-threatening to the patient, heart failure due to increased cardiac output...

"This patient's case shows the important role of regular health check-ups, not only to screen for common diseases but also to detect potential congenital abnormalities, helping with early diagnosis and treatment to avoid possible complications," Dr. Vu advised./.
