After three failed attempts to “find a child,” Ngoc Anh finally became pregnant successfully despite being diagnosed with cesarean section scar fluid retention and a uterus stuck to the abdominal wall.

Born in early April 2023, baby girl Doan Tam Anh is cute, breastfeeds, plays well, and smiles often. "She is a priceless gift to my family," Le Ngoc Anh (31 years old, from Can Tho ) shared.

Ngoc Anh got married more than 10 years ago. In 2013, she gave birth to her first son by cesarean section. Four years later, the couple decided to have another child, but she was unable to conceive. For the next seven years, Ngoc Anh and her husband traveled back and forth to Ho Chi Minh City for assisted reproductive treatment.

Two times of intrauterine insemination (IUI) and one time of in vitro fertilization (IVF) treatment at two different hospitals all failed. In May 2022, the couple went to the Center for Reproductive Support at Tam Anh General Hospital, Ho Chi Minh City (IVFTA-HCMC) for examination. In addition to irregular menstrual cycles, Ms. Ngoc Anh was found to have an abnormality in her uterus, which was also the cause of difficulty conceiving.

Doctor Le Xuan Nguyen said that the patient had long-term secondary infertility and irregular menstrual cycles. There were only about 2-3 menstrual periods a year, so the probability of sperm and egg fertilization was very low. In particular, the patient had a condition of cesarean section scar fluid retention and the uterus was tightly adhered to the abdominal wall. These were sequelae related to the previous cesarean section, leading to limitations in abnormal examinations related to fertility, reducing the success rate of assisted reproductive methods. In addition, a history of poor embryo formation also made it difficult for the patient to have a successful pregnancy.

Ngoc Anh was given a step-by-step treatment plan. Her uterus was tightly adhered to the abdominal wall and could not be examined by transvaginal ultrasound, so Dr. Nguyen had to perform transabdominal ultrasound for better results. She was stimulated once, and 24 eggs were produced, creating 8 good day 5 embryos.

Before the embryo transfer, considering and coming up with a suitable endometrium preparation regimen helped to minimize fluid accumulation in the old surgical scar, helping the uterine cavity to be clean so that the embryo could adhere well and the baby could develop healthily. Immediately after the first embryo transfer, Ngoc Anh became pregnant. The fetus was healthy, born at full term, weighing 3.8 kg. After 10 years, her family had a new member.

According to Dr. Nguyen, in addition to the patient and his wife's perseverance and determination, the method of screening for cesarean section complications, personalized treatment regimens, and an embryo culture system integrated with artificial intelligence (AI) software to help accurately evaluate embryos have increased the IVF success rate, helping patients welcome healthy children.

At IVFTA-HCMC, many cases of secondary infertility have a history of complications due to cesarean section scars. "Cesarean section scar defects are permanent. During menstruation at the beginning of each month's cycle, the menstrual flow from the uterus to the outside will pass through the cesarean section scar defect and lead to fluid retention at the old scar defect," said Dr. Nguyen. For these cases, doctors need to perform an ultrasound to assess the extent of the cesarean section scar defect, then advise the patient on the best treatment regimen.
