Connecting Electronic Health Book with VNEID: Need to unify the 3-party coordination regulations

Connecting medical data with the national database is one of the important goals of the digital transformation process in the healthcare sector. However, this process is still in the pilot phase with the bottleneck being the lack of coordination regulations among relevant parties.

No unified regulations

Each citizen will have an Electronic Health Book, linked and unified with the Personal Electronic Record. With the people's medical data on the Electronic Health Book being linked with the National Population Database, people can conveniently and easily monitor and manage their own health information continuously and for life. For medical examination and treatment facilities and related units, this brings simplification and transparency to medical examination and treatment activities, hospital fee payment, etc.

To implement the Electronic Health Book, it is necessary to carry out 3 basic roadmaps, including: Digitizing people's medical data, managing data and integrating and connecting medical data with related units. Personal electronic health books are formed from medical examination and treatment facilities. When patients come for examination and treatment, the information will be saved in the hospital management information system and form an electronic medical examination book. Thus, people do not need to use paper medical examination books anymore but use a unified electronic medical examination book at all medical facilities.

Mr. Nguyen Truong Nam, Deputy Director of the National Health Information Center (Ministry of Health) explained: According to the process, when people go for a medical examination, medical facilities must return the people's medical examination data to the Electronic Health Book (also known as Electronic Medical Examination Book/Electronic Health Record) and save it on the data system of the Ministry of Health. Based on the medical examination data, the data connection system will push the completed medical examination records to the Vietnam Social Security. The Vietnam Social Security will continue to process the data and return the file information to C06 - Ministry of Public Security to authenticate personal identification. After being authenticated by C06 - Ministry of Public Security, the file will be returned to the Ministry of Health - the unit managing the Electronic Health Book. The Ministry of Health will share this data with medical facilities and units in the localities.

Thus, in a closed cycle, information from medical facilities after going through Vietnam Social Security, C06-Ministry of Public Security will be transferred to the Ministry of Health and from the Ministry of Health will be shared with medical facilities. After people go for medical examination and treatment, localities will have a personal medical data warehouse of people to manage. At the same time, people can also grasp their own medical information through the Electronic Health Book.

When the data of the Electronic Health Book is connected with the National Database displayed on the VNEID application, people and medical examination and treatment facilities will benefit from the digital transformation of healthcare. However, Mr. Nguyen Truong Nam said that as of April 10, 2024, the connection has not been completed and is still in the pilot phase in Hanoi. Currently, technical solutions to connect and transfer data between parties have been completed. Personal records from medical facilities have been transferred to C06-Ministry of Public Security with 18 million records. Currently, C06-Ministry of Public Security is still in the process of processing data and has not returned the records to the data warehouse of the Ministry of Health. The reason is that the three parties including the Ministry of Health, Vietnam Social Security and C06-Ministry of Public Security have not signed a coordination regulation for implementation. In the coming time, when the coordination regulations are issued, the legal strictness will be ensured for the three parties to share data.

Mr. Nguyen Truong Nam added that currently, relevant units are reviewing the results and effectiveness of the pilot implementation of the use of Electronic Health Book on the VNEID application in Hanoi. Regarding the issue of coordination between the three parties, the Ministry of Health is the unit that drafted the coordination regulations so it is ready to implement. C06-Ministry of Public Security has agreed with the regulations. Vietnam Social Security is still considering.

Hanoi pilot

To connect and display health information of all people on the VNEID application, Hanoi is the pilot locality to deploy the use of Electronic Health Books on the VNEID application from November 2023 with a 3-phase roadmap.

Specifically, by the end of phase 1 (December 31, 2023), units will create electronic health books for people from the database of social insurance and health insurance participants; population data of people in the city and standardize information.

At the end of phase 2 (March 31, 2024), the units will complete updating the results of medical examination and treatment, vaccination data of people in the city who go to public and non-public medical examination and treatment facilities in the area to the Electronic Health Book database of Hanoi city, moving towards connecting and sharing health information of people in the city to the VNEID application of the Ministry of Public Security.

Phase 3 from April 1, 2024, the city police will coordinate with mass organizations to regularly review administrative information of people who have changed (missing, moved out of the area, died...) on the Electronic Health Book system.

The goal is that the entire city of Hanoi strives for 100% of people living and working stably in Hanoi to be issued with a personal electronic health book, thereby forming an electronic health record database of Hanoi city; 100% of medical health information of people in the city is monitored and managed through digital data, and at the same time shared and connected with national databases and specialized databases to serve the needs of handling work and administrative procedures of people, businesses and agencies and units in the city.

Regarding the results of the connection and transfer of data of the Electronic Health Book displayed on the VNEID application, Lieutenant Colonel Nguyen Anh Tuan, Deputy Director of the National Population Data Center, Department C06 - Ministry of Public Security, said that as of April 9, 2024, 378/673 units of Hanoi, equivalent to nearly 2 million electronic health records, had been received and transmitted to the system of Vietnam Social Security to fully synchronize 48 information fields for each individual.

Lieutenant Colonel Nguyen Anh Tuan shared that to connect data, the units agreed that the Hanoi People's Committee will proactively guide people in the city to implement. The Ministry of Health is the unit that develops general instructions on the steps in the data connection process nationwide. The instructions are expected to be issued by the Ministry of Health in June 2024. To speed up the progress of data connection in the coming time, the Ministry of Health needs to soon issue instructions on documents to synchronously deploy the Electronic Health Book nationwide; continue to urge hospitals to continue to synchronize data to the Vietnam Social Security system to display on the VNEID application.

To speed up the progress of data interconnection, Deputy Minister of Health Tran Van Thuan has directed the Department of Medical Examination and Treatment Management (Ministry of Health) to urgently complete the regulations on personal health information fields displayed on the pilot VNEID application in Hanoi as a basis for nationwide deployment. The Deputy Minister of Health also requested the Department of Science, Technology and Training (Ministry of Health) to coordinate with the National Health Information Center, relevant Departments, Agencies and units to complete the Regulations on data sharing coordination and coordination between the Ministry of Health, the Ministry of Public Security and the Vietnam Social Security to implement Project 06 of the Government, promote the connection of Electronic Health Books to the VNEID application, contributing to the implementation of the digital transformation goal of the health sector.
