According to a preliminary report from the Ho Chi Minh City Department of Food Safety, "Miss Bich's Toad Bread" registered its business in 2020 at 112A Nguyen Thai Son, Hanh Thong Ward.
This facility is about 20 square meters wide, has 1 stainless steel bread cart, 3 refrigerators, 1 freezer to store food.
There is also branch 2 in Binh Loi Trung ward (HCMC) but it is not registered for business.
The owner said he bought the ingredients and processed them himself.
Specifically, each day, the facility buys about 50-60kg of chicken breast from an address on Le Duc Tho Street, without a purchase invoice. Then, it processes it at home and uses it all up within the day.
The butter is made from eggs, cooking oil and lemon; about 120 eggs are bought every day from a facility on Dien Bien Phu Street, without a purchase invoice. The pickles are bought from a small facility, without a receipt, washed, soaked in salt and sold out within the day.
Pate bought at a store on Nguyen Thien Thuat Street, no receipt. Bread bought at a store on Pham Ngu Lao Street. As for the pork roll, there was a receipt for 35kg purchased on November 5 from a business on Pham Van Chieu Street.

The report of the Department of Food Safety stated that this was a food poisoning case with a large number of infected people, some cases progressed to severe illness, the incident occurred in the community, and the number of people who ate the bread could not be specifically identified.
In addition, the investigation teams only recorded cases from treatment facilities, making it difficult to compile statistics and analyze epidemiology.
The Department of Food Safety recommends that the Ho Chi Minh City People's Committee direct departments, branches and localities to support and coordinate the investigation and handling of food poisoning.
Regarding the progress of handling the case, after receiving information from people via email about a suspected food poisoning case, on November 7, the department established a working group to investigate and handle the case.
On the afternoon of November 7, the Department of Food Safety and the People's Committee of Hanh Thong Ward sealed the sample cabinets containing ingredients at the "Miss Bich's Toad Bread" facility on Nguyen Thai Son Street.
On the morning of November 10, the investigation team sent samples of ingredients including pate, chicken, avocado, pickles and cucumbers to test for the agent causing food poisoning.
As of noon on November 12, 316 people in Ho Chi Minh City were hospitalized due to digestive disorders after eating bread from the "Miss Bich's Toad Bread" establishment.
Based on test results, the Ho Chi Minh City Department of Health determined that the agent causing food poisoning in the incident could be salmonella bacteria (from blood and stool cultures).
