A patient with a rare bilateral main bronchus rupture has just been miraculously saved.

(Chinhphu.vn) - An 18-year-old female patient suffered from a traffic accident, multiple injuries, including ruptured main bronchi on both sides. This is the first case of bilateral main bronchi rupture recorded in Vietnam and one of the extremely rare cases in world medical literature, and has just been miraculously saved at Viet Duc Friendship Hospital.

Speaking to reporters, Associate Professor, Dr. Pham Huu Lu, Deputy Head of the Department of Cardiovascular and Thoracic Surgery, Cardiovascular and Thoracic Center, Viet Duc Friendship Hospital, said that on April 8, patient PTTN (18 years old, Hung Yen ) was transferred to Viet Duc Friendship Hospital in critical condition after a serious traffic accident between a motorbike and a car. The patient was directly run over by the car wheel.

Previously, the patient received first aid at Hung Ha General Hospital, was intubated, had bilateral pleural drainage, and was urgently transferred to Viet Duc Friendship Hospital.

Immediately after receiving the patient, the entire emergency system of Viet Duc Friendship Hospital was immediately deployed throughout the hospital, from clinical to paraclinical diagnosis, especially emergency bronchoscopy.

The final results showed that the patient had bilateral main bronchi rupture. This is a very serious and rare injury with a very high mortality rate, directly threatening life due to severe respiratory and hemodynamic disorders.

Patient returned for follow-up visit 1 month after discharge from hospital

The patient also suffered from traumatic brain injury, facial deformity, mandibular fracture, and grade 2 liver and spleen injuries.

Associate Professor, Dr. Pham Huu Lu said that, faced with the critical situation, the hospital quickly held a consultation and decided on emergency treatment.

According to experts, this is the first case of bilateral main bronchus rupture recorded at Viet Duc Friendship Hospital and Vietnam, and is also one of the extremely rare cases in world medical literature, with a rate of less than 1% in severe multiple trauma cases.

"If one main bronchus ruptures, the patient's danger level is less, but in this case, both main bronchus rupture, so the entire airway is blocked. If emergency treatment is not done properly or promptly, the patient can easily fall into a state of brain death due to lack of oxygen, leading to death," said Associate Professor Pham Huu Lu.

After treatment, re-establishing bilateral airway circulation, the patient's condition stabilized (arterial blood oxygen saturation 98 - 99%, stable hemodynamics). The patient was transferred to the intensive care unit for treatment.

However, during post-operative care, patients still face other major risks such as: bilateral lung infection; risk of rupture or leakage of the bronchial anastomosis; how to ensure nutrition, prevent wound ulcers, and ensure regular bronchoscopy for patients...

Fortunately, with experience in performing 7 lung transplants, the doctors at Viet Duc Friendship Hospital are very proficient in caring for the patient. After three weeks of intensive treatment, the patient's surgical wounds recovered well, the patient was conscious, the endotracheal tube was removed and he was transferred to the Department of Cardiovascular and Thoracic Surgery, Cardiovascular and Thoracic Center, Viet Duc Friendship Hospital for further monitoring.

After that, the patient received maxillofacial rehabilitation treatment (combining the lower jaw bone with screws and splints).

Four weeks after the accident, the 18-year-old girl was discharged from the hospital in stable condition. The patient had overcome one of the most severe challenges of her life.

During a follow-up examination 1 month after discharge from the hospital last week, Associate Professor Pham Huu Lu said that the patient had almost completely recovered with a cheerful physical condition and optimistic spirit. In particular, the results of the bronchoscopy after repair showed that the anastomoses between the two main bronchi were very good, with no signs of stenosis or leakage.

Other organs recovered well to the joy and excitement of the family and medical staff involved in emergency care and treatment of the patient.
