Previously, on April 12, pregnant woman NTXN (35 years old, living in Phu Yen ) gave birth to her second child, and was monitored at a lower level. In the early morning of April 13, the pregnant woman had difficulty breathing, chest tightness, suddenly lost consciousness, and quickly suffered respiratory and circulatory failure. She was diagnosed with shock due to suspected amniotic fluid embolism. The patient was transferred to the emergency room at Phu Yen Obstetrics and Pediatrics Hospital in an extremely critical condition, with severe shock and unmeasurable pulse and blood pressure.
Doctors at Phu Yen Obstetrics and Pediatrics Hospital immediately replaced blood fluids, used vasopressors... to rescue the patient and performed timely assessment tests. The fetal heart rate was erratic, there was a risk of the fetus being lost in the womb. Test results showed that the patient had a very severe blood clotting disorder; Phu Yen Obstetrics and Pediatrics Hospital immediately consulted with Phu Yen General Hospital to coordinate treatment.
The doctor explained to the pregnant woman's family the condition, which required emergency surgery and a high risk of death. The surgery was successful, delivering a baby boy weighing 3.4 kg, who was resuscitated and transferred to the Neonatal Pediatrics Department for monitoring and treatment. For patient N, who was in a state of severe blood clotting disorder, the surgical team persevered in stopping the bleeding and preserving the uterus with experience in difficult surgical techniques; the recorded blood loss during the surgery was 1.7 liters, or nearly half the average blood volume of an adult. After the critical period, the pregnant woman regained consciousness, escaped shock, regained consciousness and continued to be monitored and treated at Phu Yen General Hospital from the evening of the same day.
Amniotic fluid embolism is the most feared complication in obstetrics, because it progresses rapidly, is very unexpected, cannot be prevented, and the mortality rate is up to more than 80% due to respiratory failure, circulatory collapse, and blood clotting disorders. Preserving the uterus or not is always a big challenge for surgeons; correcting blood clotting disorders is a difficult problem because severe blood clotting disorders can easily lead to multiple organ failure and death - which is very common in amniotic fluid embolism.
