Doctor Pham Minh Ngoc, Deputy Director of the Hanoi Center for Gender Medicine, said that wrong habits in relationships not only affect physiological health but can also impact psychology and quality of life.

Swollen tonsils, detected stage 3 lymphoma

Ms. CTV (72 years old, Tay Ninh ) had a sore throat for a month that did not go away. Her right tonsil was swollen to the size of an apple, causing her to have difficulty swallowing and speaking. She took medicine but it did not improve. For many days, she could not eat or drink, she vomited as soon as she ate; her stomach was bloated and tense.

The CT scan results showed that Mrs. V. had many lymph nodes measuring 34x21mm on both sides of her neck. There were many tumors in her abdomen, the largest of which was 8x13cm, adhering to the duodenum, causing duodenal obstruction, gastric fluid retention, adhesion of the head of the pancreas, and surrounding the abdominal aorta. This was the reason why Mrs. V. had the above symptoms.

According to oncologist Nguyen Tran Anh Thu, biopsy of the tonsil tumor and pathological examination showed non-Hodgkin lymphoma, stage III because of lesions in many locations: cervical lymph nodes, axillary lymph nodes, abdominal lymph nodes, and right tonsil.

After performing evaluation tests, Ms. V. was given chemotherapy to reduce the size of the lymph nodes. Ms. V. responded well to the treatment.

After the first course of medication, the lymph nodes in her abdomen shrank and no longer compressed the surrounding organs; her right tonsil also returned to its normal size. Ms. V. was able to eat and drink, no longer had painful swallowing, no longer vomited, and her abdomen was no longer tense.

Doctor Anh Thu said the patient needs 5 more chemotherapy sessions to complete the treatment, helping to reduce the size of the lymph nodes and prevent the lymphoma from recurring.

Lymphoma is the uncontrolled division of lymphocytes (an important part of the immune system, which helps protect the body against infections and many other diseases). The disease can occur at any age, but is more common in men aged 50 and older.

Lymphoma consists of two main types: non-Hodgkin lymphoma (accounting for about 90%) and Hodgkin lymphoma. Non-Hodgkin lymphoma has a poor prognosis and is more difficult to treat than Hodgkin lymphoma. However, the disease can be completely controlled even in stages III and IV, as in the case of Ms. V.

The 5-year survival rate of patients with non-Hodgkin lymphoma depends on certain factors such as stage, type of cancer, age, response to treatment, etc.

According to the International Agency for Research on Cancer and the Surveillance, Epidemiology, and End Results (SEER) Program of the National Cancer Institute in 2023, the survival rate of people with non-Hodgkin lymphoma is 90% in stage 1; 78% - 90% in stage II; 50% - 78% in stage III and 40% - 50% in stage IV.

According to statistics in Vietnam in 2022, non-Hodgkin lymphoma recorded more than 3,500 new cases and was the cause of death for more than 2,200 people. The high mortality rate is due to patients coming to the hospital at a late stage.

Dr. Anh Thu said that the cause of lymphoma is currently unknown. However, the disease has some risk factors such as genetic immune deficiency, use of immunosuppressive drugs, HIV infection, exposure to toxic chemicals (pesticides, etc.). Having hepatitis B, hepatitis C, Epstein-Barr, herpesvirus 8, helminth infections, autoimmune diseases, smoking, poor diet and prolonged stress.

Although lymphoma can occur at any age, it usually appears in the elderly and people with weakened immune systems. However, the cure rate for early-stage lymphoma is quite high if detected and treated promptly.

Pectus excavatum: Early surgery improves children's health and psychology

Hung was born with a sunken chest, a condition that developed after puberty. He often feels out of breath during strenuous exercise, which can affect his heart and lung function.

The patient showed signs of fatigue and difficulty breathing when playing sports, and the indentation became more obvious after puberty. This condition can worsen, leading to compression of the heart and lungs, scoliosis, and causing low self-esteem in children. The doctor prescribed a Nuss surgery with thoracoscopy to lift Hung's sternum.

For orthopedic surgeries involving bones and the chest, the patient will be given erector spinae plane (ESP) anesthesia. Pectus excavatum surgery is very painful during and after surgery because the chest is suddenly lifted by metal bars, the muscles, bones, and ligaments are stretched, causing severe pain after waking up from anesthesia.

Dr. Nguyen Do Trong, Department of Pediatric Surgery, Tam Anh General Hospital, Ho Chi Minh City, said that pectus excavatum is a common congenital defect that occurs when there is excessive or unbalanced development of the lower rib cartilages, causing the sternum to be pushed inward, causing pectus excavatum. Children are often diagnosed with this defect right after birth or during puberty.

For most children with pectus excavatum, the only noticeable sign is a slight indentation in the chest. The indentation becomes more apparent as the child enters adolescence, and the condition worsens as they grow older.

Mild pectus excavatum is usually asymptomatic and does not affect health. However, if left untreated, the disease can cause pain due to bone deformity, muscle tension or compression of the heart and lungs, pushing the heart to the left side of the chest, reducing the ability to pump blood, affecting the child's physical activities. If not intervened early, the disease can also affect the child's long-term psychological development.

In severe cases, children often have symptoms such as rapid heartbeat, wheezing, cough, chest tightness, fatigue, dizziness, shortness of breath, weakness during exercise, and recurring respiratory infections.

Doctors recommend that parents monitor their children's health to detect early symptoms of the disease, take them to the doctor and get timely treatment. Surgery helps limit complications, improve children's health, aesthetics and psychology.
