The patient, Ms. D.TP (38 years old), was admitted to the hospital due to severe pelvic pain below the navel accompanied by bloody stools after using a coffee enema at a private clinic.

The patient had previously used this method twice, one week apart. On the third occasion, during the enema, the patient experienced severe pain in the lower abdomen and bleeding through the anus. Doctors decided to perform emergency surgery to treat the injury after discovering a collection of gas in the retroperitoneal space, suspecting a rectal rupture.
During the surgery, the doctors noted a ruptured rectum in the lower third, causing a retroperitoneal abscess. In addition to suturing the rectal rupture, the doctors performed extensive drainage of the retroperitoneal space and created an artificial anus for the sigmoid colon. It is expected that in a few months, Ms. P. will have to undergo another surgery to close the artificial anus.
