Building a grassroots health system suitable for the 2-level local government model

To reorganize and reorganize administrative units at all levels and build a 2-level local government model, the Ministry of Health is drafting a draft of the functions, tasks, and organizational structure of the grassroots health system accordingly. The draft is assessed to have many new points, demonstrating the motto of innovation, streamlining, improving the quality, and effective operation of the grassroots health system.

Propose 4 new job positions

Implementing Conclusion 127-KL/TW of the Politburo and the Secretariat on studying and proposing the organization of the political system, the Ministry of Health has drafted the functions, tasks, and organizational structure of the grassroots health system in building a 2-level local government organization model. Currently, the draft guidance on job positions, functions, tasks, powers, and organizational structure of the grassroots health system is being widely consulted in health units. The draft is assessed to have many new points, demonstrating the motto of innovation, streamlining, improving the quality, and effective operation of the grassroots health system.

Accordingly, it is expected that health centers will be established in communes and wards as public health service units with legal status, headquarters, seals and separate accounts. Health centers in communes and wards have 17 tasks and powers. The functions of these health centers in communes and wards are to provide professional and technical services in preventive medicine; primary emergency examination and treatment; rehabilitation, health care for mothers and children, the elderly, ensuring food safety; providing medicines, vaccines, basic medical equipment, population services, social protection, social assistance and other health services according to the provisions of law in the communes and wards under their management.

Dr. Nguyen Thi Ha Sinh, former Director of the Provincial Department of Health from 1994 to 2001, said: The proposed organizational structure of the People's Committee at the grassroots level (commune, ward) has 4 specialized departments, including the Department of Culture and Society , which is a specialized agency advising the People's Committee to perform the function of state management in the fields of culture and society, including the fields under the management of the health sector. To perform the function of advising the People's Committee at the grassroots level, the Ministry of Health proposed to add 4 civil servant positions under the Department of Culture and Society, which in my opinion is a proposal for innovation, as a basis for recruitment, promotion, and transfer of civil servants. The 4 proposed additional positions include: Medical professional management; management of pharmaceuticals, medical equipment, food safety; population management, child care, elderly care and social protection management position. These are important positions, a bridge to bring health policies and laws to the people.

Reorganization of multifunctional medical center

Currently, district, town and city health centers are multi-functional health units. For the inpatient examination and treatment block, it is planned to reorganize into regional general hospitals under the Department of Health, or into a second facility of a provincial general hospital to organize basic medical examination and treatment for people in the area, regardless of administrative boundaries. However, the establishment, arrangement and reorganization of regional general hospitals must depend on the medical examination and treatment needs of people in the area, the geographical characteristics of the area and the planning of competent authorities.

For departments and offices of preventive medicine in district, town and city health centers, they will be reorganized into commune and ward health centers. District health centers, which only perform preventive medicine functions, will be reorganized into grassroots health centers (commune and ward levels) for new communes and wards according to the new organizational model.

Regarding the current plan to arrange commune and ward health stations, Mr. Nguyen Hong Son, Director of the Department of Organization and Personnel (Ministry of Health), said: “The existing commune and ward health stations will choose the commune and ward health station as the center of the grassroots administrative unit (new commune and ward) to reorganize into commune and ward health centers. The sector plans to mobilize human resources, doctors and nurses from the previous district health centers and from commune and ward health stations in the area to work at the commune and ward health centers of the new administrative unit.

The remaining commune and ward health stations are organized into stations to manage, monitor, provide first aid, and vaccinate local residents and residential areas. For grassroots administrative units in special zones and island districts, the current health centers in special zones will be maintained and reorganized into grassroots health centers according to the 2-level local organization model. However, inpatient examination and treatment can be organized at grassroots health centers in special zones.
