Accordingly, after the meal at Tue Duc Primary and Secondary School, many students showed symptoms of nausea, stomachache, and diarrhea. The school meals were provided by Haxeca Mekong International Joint Stock Company, so the Food Safety Department determined that the suspected poisoning cases occurred at the same time, using the same food groups, and needed to be thoroughly investigated.

The Food Safety Department requested the Ho Chi Minh City Food Safety Department to trace the origin of food, identify the unit supplying raw materials and processing suspected food poisoning; take food samples and specimens for testing to clarify the cause; temporarily suspend the processing facility if it is found to be at risk of further poisoning; strictly handle food safety violations and publicize the results to warn the community.
The Food Safety Department also requested the Ho Chi Minh City Department of Food Safety to strengthen management at collective kitchens and ready-to-eat meal providers, requiring strict control of raw materials, processing and transportation procedures, performing 3-step food inspection and storing samples according to regulations.
According to a report from Tue Duc Primary and Secondary School, on the morning of March 26, the homeroom teacher of class 1P6 received information from the parents that 2 students had symptoms of nausea and diarrhea, were taken to the hospital and diagnosed with suspected food poisoning. After breakfast that same day, the school recorded 2 more students with similar symptoms.
At noon the same day, the Board of Directors held an urgent meeting with relevant departments and compiled a list of 38 students with suspected signs of poisoning. The school took food samples and sent them to the Pasteur Institute in Ho Chi Minh City for testing, and at the same time notified parents to coordinate in monitoring the health of the students.
