Continuously saving the lives of fetuses with twin-to-twin transfusion syndrome

A pregnant woman living in Lao Cai, 18 weeks and 4 days pregnant with twins, one placenta and two amniotic sacs, was diagnosed with stage II twin-to-twin transfusion syndrome. When arriving at Hanoi Obstetrics Hospital, ultrasound consultation showed that the right fetus (donor fetus) had no visible bladder, and the small intestine had increased echogenicity.

Meanwhile, the left fetus (the fetus receiving blood) has a three-vessel umbilical cord, with the umbilical cord root attached to the center of the placenta. This is a complicated case, requiring timely intervention to balance blood circulation between the two fetuses.

Another case was a pregnant woman with 16-week-old twins, a diamniotic placenta, diagnosed with twin-to-twin transfusion syndrome in which one fetus had no heartbeat.

The ultrasound results showed that the heartless mass had no head, had a chest, abdomen and two lower limbs but no fetal cardiac activity was observed. Meanwhile, the pumping fetus (donor fetus) was smaller than the actual gestational age, however, the doppler did not detect any abnormalities.

This is a rare condition and can have serious consequences for the fetus if not treated promptly.

Faced with the dangerous condition of both mothers, the team of doctors at Hanoi Obstetrics and Gynecology Hospital quickly performed laparoscopic laser photocoagulation surgery on the uterus.

Using advanced techniques, doctors severed the abnormally connected blood vessels on the surface of the placenta, helping to stabilize blood circulation between the two fetuses. After surgery, the health of the mother and fetus were stable, and the twin-to-twin blood transfusion was completely controlled.

Thanks to the application of advanced techniques in the treatment of high-risk pregnancies, many babies have had the opportunity to be born healthy at Hanoi Obstetrics Hospital, bringing boundless happiness to families.
