Successful urethroplasty for 65-year-old patient

On June 20, Binh Dan Hospital (HCMC) announced that doctors from the hospital's Urethral Reconstruction Unit performed urethral reshaping surgery on a 65-year-old male patient.

This is the case of male patient NVN ( Long An ). In 2022, the patient had to undergo retrograde endoscopy to crush urethral stones. Previously, the patient had symptoms such as frequent urination with burning pain in the urethra and had an endoscopy performed at a local hospital. However, the patient still had persistent difficulty urinating, which progressed to the point of complete urinary retention. Therefore, the patient had surgery to open the bladder to the skin to drain urine. In addition, the patient also had underlying diseases including gout and hypertension.

On the morning of June 20, doctors from the Urethral Reconstruction Unit of Binh Dan Hospital and a delegation of Professor Joel Gelman (UC Irvine University, California, USA) performed urethral reconstruction surgery for the patient.

The surgery to reconstruct the patient using the end-to-end urethral anastomosis method was broadcast live from the operating room so that the attending doctors and medical staff could learn from experience in handling surgical situations.

It is known that since 2015, Professor of Urology Joel Gelman has transferred plastic surgery techniques for patients with urethral stricture and provided professional support to Binh Dan Hospital.

Professor Joel Gelman said that the success rate of urethroplasty surgery at this hospital is 98%, equivalent to the results of leading urethroplasty centers in the United States.

Since then, Binh Dan Hospital has not only transferred this technique to domestic hospitals but also to hospitals in Southeast Asian countries such as Thailand and Malaysia.

Dr. Do Lenh Hung - Deputy Head of Department of Urology C, Head of Urology Unit, Binh Dan Hospital - said that the number of cases of urethral strictures being treated at Binh Dan Hospital is increasing. Currently, each year, the hospital receives more than 800 surgeries for urethral strictures and examines and treats about 10,000 other cases.

"The most important thing is that the patient is treated right from the first time, correctly for us. As for cases of previous treatment failures, the more failures, the lower the success rate. It is not a technical problem anymore, but the problem is that the tissues have been damaged, the blood supply is lacking, so the chance of success is reduced," said Dr. Do Lenh Hung, MD.

According to experts, urethral stricture in Vietnam and some Southeast Asian countries often occurs after pelvic fractures when patients have traffic accidents, especially motorbike accidents, falls from sitting on the side of boats and work accidents.

Urethral stricture causes difficulties in the patient's life due to the condition of straining to urinate, difficulty urinating, and even needing emergency treatment for urinary retention. Urethral stricture disease leaves serious psychological consequences because it causes disorders of the lower urinary tract, along with complications of urinary tract infections. This not only reduces the patient's quality of life, but also increases the burden on society when the patient group is mainly of working age.

If urethral stricture treatment is not performed properly, it can cause recurrence of stricture, requiring multiple surgeries or requiring the patient to wear a catheter for life. In countries without well-trained doctors and specialized equipment for urethral reconstruction, the success rate is often less than 16%.

According to Dr. Do Lenh Hung, over the past 8 years since the establishment of the Urethral Unit, the number of surgeries performed by the unit's doctors has increased each year. Previously, the hospital performed plastic surgery for about 250 cases of urethral strictures per year, then gradually increased to more than 500 cases. Currently, about 800 cases per year are treated at Binh Dan Hospital.

The doctor advised the patient that the results of urethroplasty should be closely monitored. A successful urethral stricture surgery should be re-evaluated after 3-4 months. The patient should also have a follow-up visit every year as scheduled by the doctor to ensure that the blood supply to the reconstructed area is still good, avoiding the risk of re-stricture.
