On the afternoon of May 16, truck with license plate 51D-764.96 was traveling on Nguyen Van Luong Street (from Hau Giang Street to Vo Van Kiet Street). When it reached the Pham Van Chi Bridge underpass, Ward 10, District 6, despite discovering a 2m height limit sign attached to the barrier, the driver still deliberately drove the vehicle into the underpass.

The truck cabin passed through the barrier but the rear part of the truck was stuck. At the scene, part of the truck's cargo bed was deformed and the height restriction barrier collapsed. The District 6 Traffic Police Team, Traffic Police Department, and Ho Chi Minh City Police were present at the scene to handle the incident.

The Pham Van Chi bridge area in previous years was a black spot for traffic congestion due to vehicles on Nguyen Van Luong, Pham Van Chi, Ly Chieu Hoang streets... coming here and clashing with each other, especially during rush hour. The slope of Pham Van Chi bridge, ward 10 side has a high slope, many vehicles pass through here, especially when it rains, it is slippery and many serious traffic accidents have occurred.

In 2019, the 150m long, 8m wide underpass with 2 lanes from Nguyen Van Luong Street to Lo Gom Bridge and vice versa was completed and put into use, resolving conflicts between traffic flows in this area, eliminating traffic congestion. At both ends of the underpass, a 2m height limit bar was installed to prohibit trucks and tractor-trailers.
However, since its inception, dozens of trucks have deliberately driven into the tunnel and crashed into the barrier. In many cases, drivers did not observe the barrier and drove trucks at high speed, crashing into the barrier, causing the truck cabin to deform and the truck bed to fly out of the vehicle.
