Previously, patient HVD, in Ben Quan commune, Quang Tri province, had a traffic accident and was brought to the emergency room with the following conditions: Closed fracture of the right femur; open fracture of the upper end of the two right tibiae; loss of peripheral pulses. Initial examination results showed that the blood vessels of the lower limbs had lost signal of the popliteal artery and anterior tibial artery, predicting that the patient was at risk of amputation and systemic poisoning due to prolonged leg ischemia.
Immediately, doctors from the Department of Intensive Care and Anti-Poison and the Department of Trauma and Burn Surgery held a consultation and performed the first surgery lasting more than 4 hours to fix the femur with a peripheral fixation frame; free the calf compartment to assess the survival of the muscles; explore the popliteal artery and connect the tendons of the left hand.
However, the results of the examination at this time still showed that the artery was still stuck in the middle of the fractured tibia. In addition, the foot was still cold and purple due to poor blood circulation in the lower leg and the contusion and blockage of the popliteal artery and the anterior tibial artery, nearly 20cm long. The surgical team decided to perform a vascular bypass surgery to restore blood flow to the lower limb by taking a 40cm long section of the left leg vein to act as a bridge, replacing the damaged popliteal artery, and re-opening blood flow from the thigh to the lower leg. The success of the major surgery was to save the young patient's legs. Up to now, the patient has recovered and is in completely stable health.
It is known that popliteal artery injury is relatively rare, 5-19% in limb vascular injuries, the rate increases in cases of trauma around the knee joint; if not treated promptly, the amputation rate can be up to 30-60%. The success of the surgery is a testament to the expertise, sense of responsibility and the tireless efforts of the medical team of Quang Tri General Hospital in the journey to save lives.
