A 36-year-old woman was admitted to the hospital about 3 hours after giving birth at home in a state of severe hemorrhagic shock, unmeasurable blood pressure, and deep coma due to placental abruption and massive bleeding.

Can Tho City Obstetrics and Gynecology Hospital coordinated with Can Tho City General Hospital to successfully intervene in emergency care for a pregnant woman in critical condition due to postpartum complications at home.

Previously, on June 23, Ms. Dang Thi Ng (36 years old, permanent resident of Mai Dam town, Chau Thanh A district, Hau Giang province) was hospitalized about 3 hours after giving birth naturally at home in a state of severe blood loss shock, unmeasurable blood pressure and deep coma due to placenta not separating and massive bleeding.

The family said this was Ms. Ng's third birth. During her pregnancy, she only had one check-up.

After giving birth at home, the family found that the placenta had not detached and the mother was bleeding heavily, so they asked a nearby doctor for help. However, the situation was beyond their ability to handle, so Ms. Ng received first aid and was asked to be transferred to an emergency hospital.

On the way to the hospital, the pregnant woman continued to bleed and showed signs of coma, and was taken straight to the Emergency Resuscitation Department, Can Tho General Hospital.

Immediately, the emergency team of Can Tho General Hospital activated the internal red alert procedure and invited consultation and emergency coordination from Can Tho Obstetrics Hospital.

Doctors from Can Tho General Hospital were in charge of resuscitation and blood transfusion. Obstetricians and Gynecologists from Can Tho Obstetrics and Gynecology Hospital performed artificial removal of the placenta, controlled the uterus and performed high-tech hemostasis.

Thanks to timely and accurate professional intervention, the mother's health has stabilized and she is currently being monitored in the intensive care unit of Can Tho General Hospital.

According to specialist doctor II Do Thi Minh Nguyet, Deputy Head of the Department of Obstetrics and Gynecology (Can Tho City Obstetrics and Gynecology Hospital), the risk of complications when a mother gives birth or gives birth at home is very high. Dangerous situations that are easy to encounter are trauma, vaginal tears, vulva and perineum tears, retained placenta, postpartum hemorrhage, uterine atony, hemorrhagic shock, postpartum infection...

The fetus may experience complications such as: asphyxia and amniotic fluid aspiration, placental bleeding, fetal trauma, hypothermia and infections, especially conjunctivitis, pneumonia...

To control childbirth and avoid possible difficulties and risks, Dr. Nguyet recommends that pregnant women should have regular check-ups and monitor their pregnancy on schedule to detect abnormalities early; at the same time, they should work and rest properly, avoid overexertion and excessive stress; and eat a nutritious diet with a variety of foods.

When there are unusual signs, pregnant women need to be hospitalized early at a specialized hospital or medical facility with professionally trained doctors and nurses to ensure the safety of mother and child./.
