After receiving emergency care at the local hospital, Mr. T. was transferred to Viet Duc Friendship Hospital at the 12th hour after the injury.

The patient was admitted to the hospital with a wound that was almost severed from the penis, the damaged part was still pink, the glans was tense; the corpus cavernosum and corpus spongiosum were completely severed; the urethra of the glans penis was crushed; the wound peeled off the scrotal skin, exposing both testicles, however, the testicles were not damaged and the dorsal penile neurovascular bundle was still present.

After examination and diagnosis, the doctor performed sutures to connect the corpus cavernosum and corpus spongiosum, and created scrotal skin to cover both testicles.

After 2 days of surgery, the penis was preserved pink, well perfused, the incision was dry, had little fluid absorption, and showed no signs of infection.

Mr. T. was regularly examined by the doctor after surgery and given a treatment regimen to monitor erectile recovery later.

Master, Doctor Bui Van Quang, Center for Andrology, Viet Duc Friendship Hospital, said this was a rare and unusual accident but left behind extremely complicated injuries.

The victim's penis was crushed, making it difficult to treat and preserve the severed penis.

Dr. Quang recommends that people should be careful and ensure occupational safety to avoid unfortunate accidents. In case of an accident, first aid should be given, the severed part should be properly preserved and the patient should be transferred to a specialized medical facility for timely treatment.

When handling a severed part, stay calm, put the severed piece in a clean plastic bag containing saline (bottled saline used in medicine, not homemade saline) or clean water, then put it in a box or bag of ice (not directly in ice water) to avoid frostbite to tissue and blood vessels.

Do not ligate or cauterize during first aid at lower-level facilities (to avoid damaging blood vessels) unless absolutely necessary. Quickly transfer to a medical facility that can handle the situation to avoid wasting time.
