Pregnant woman VTM, 40 years old (from Viet Tri, Phu Tho ), is pregnant for the fourth time with a history of 3 cesarean sections, was diagnosed with gestational diabetes and is being treated with insulin every day. This pregnancy is a difficult journey as she was also diagnosed with central placenta previa and placenta accreta - one of the most dangerous complications in obstetrics, which can threaten the mother's life at any time if not treated promptly.
Recognizing this as a very high-risk case, doctors at Hanoi Obstetrics and Gynecology Hospital proactively developed an optimal intervention plan and planned a cesarean section at week 36.
The surgery was performed by a highly specialized team led by Master, Specialist II Doctor Do Khac Huynh - Deputy Director of the hospital; Specialist II Doctor Vuong Duc Hinh; Anesthesiologist, Specialist II Doctor Luong Thi Ngoc Van - Deputy Head of Anesthesiology and Resuscitation Department and the team.
When opening the abdomen, the entire potential risk of the pregnant woman is clearly shown when the abdomen is heavily adherent, the front of the uterus is firmly adherent to the abdominal wall, and the bladder is adherent as high as the body of the uterus. These are serious signs of placenta accreta.
The doctors were focused, racing against time to save both mother and child. Every cut was a precise decision, every second was a race against time and life limits.
After welcoming the 3,800g baby boy into the world, the doctors entered the race to save the mother's life. The doctors immediately performed a hysterectomy to control bleeding, ligated the bilateral hypogastric arteries, maximally preserved physiological functions, and ensured the mother's life. The mother was transfused with a total of 1,700ml of packed red blood cells, 300ml of cryoprecipitate, and 1,000ml of plasma during the intervention.
The surgery lasted several hours. After the surgery, the mother was closely monitored in the Anesthesia and Resuscitation Department and her condition was stable.
