Surgery to remove a giant uterine fibroid that spread to the lower flank

Hanoi Obstetrics and Gynecology Hospital has just admitted a 51-year-old female patient residing in Hanoi, who came for examination with a tumor in the lower abdomen spreading from the right pelvic wall up to the lower ribs.

Through examination, the A5 gynecologists initially determined that this was a solid tumor, most likely originating from the uterus, very large in size, about 30cm, changing the structure of the pelvis.

To accurately assess the location, size and characteristics of the tumor, the patient was ordered to have an MRI scan of the pelvis and abdomen.

After consultation, Dr. Le Thi Anh Dao, head of the A5 gynecology and surgery department, and Dr. Tran Van Cuong, head of the anesthesia and resuscitation department, and the team performed surgery on the patient.

During the surgery, the team noted that this was a L8 uterine fibroid, growing from the pelvic area up to the right hypochondrium, nourished by a large vascular pedicle. This is a special location, with the potential risk of heavy blood loss.

Determining that this was a tumor with a high risk of bleeding and altering the pelvic anatomy, with a risk of damaging the ureter, the surgical team invited Dr. Do Khac Huynh - Deputy Director of the hospital in charge of professional matters - to participate in the surgery.

Thanks to meticulous surgical techniques, the doctors were able to preserve the ureter and bladder, and at the same time ligated the right hypogastric artery to limit bleeding. The surgery went smoothly, with little blood loss and no need for a blood transfusion. The removed uterine fibroid was about 30cm in size and weighed 3.2kg.

Doctors recommend that gynecological diseases can progress silently over a long period of time without obvious symptoms. Therefore, women should have regular gynecological examinations, especially when there are unusual signs to be detected and treated early, avoiding complicated complications.
