Keeping a life in the uterus for a pregnant woman carrying twins in rare different locations

In the 5th week of pregnancy, the pregnant woman was found to have one fetus in the uterus and one ectopic pregnancy that had ruptured and caused abdominal bleeding. The doctors tried to remove the ectopic pregnancy and keep the remaining life of the pregnant woman.

On September 29, the Emergency Department of the Central Maternity Hospital received patient D.T. (born in 1995, from Vinh Phuc ) in a state of severe abdominal pain in the left iliac fossa, fatigue, and dizziness.

Ultrasound showed a gestational sac in the uterus about 5 weeks old, at the same time there was a lot of bloody fluid in the abdomen, suspected of ruptured left ectopic pregnancy. Recognizing this as a dangerous emergency situation, the doctors consulted and ordered an emergency cesarean section.

The woman had no children, had two miscarriages – one at 8 weeks, and one due to an abnormal pregnancy (Hygroma Kystique - Pulmonary artery stenosis). After many years of treatment, in August 2025, she was stimulated to ovulate, combined with natural intercourse, and the joy came when she became pregnant after many days of waiting.

When she was about 5 weeks pregnant, she began to feel a dull pain in the left iliac fossa. Initially, at the local medical facility, she was diagnosed with ovarian hyperstimulation and monitored internally. But after only a few days, the pain became more and more intense, forcing her to be transferred immediately to the Central Obstetrics Hospital.

Advanced ultrasound results determined that the twins were in different locations – one in the uterus, one ectopic pregnancy that had ruptured and caused abdominal bleeding.

Upon admission, the patient was urgently consulted by Dr. Dao Thi Hoa, Head of the Examination Department. The surgical team led by Dr. Pham Minh Giang, Specialist II, as the main surgeon, quickly performed emergency surgery.

During the surgery, the team discovered about 600ml of thin blood and blood clots, the left fallopian tube had ruptured, and was bleeding into the abdominal cavity. The doctor cut the left fallopian tube, controlled the bleeding thoroughly, drained the blood and washed the abdominal cavity, while minimizing the impact on the uterus to avoid affecting the fetus in the uterine cavity.

Thanks to early detection and timely treatment, the patient survived the critical stage, and the remaining fetus in the uterus continued to develop normally.

Specialist II Doctor Pham Minh Giang shared that this is a special case where the patient is pregnant in the uterus and also in the left fallopian tube that has ruptured, causing abdominal bleeding. These cases are rare and sometimes difficult to diagnose, requiring doctors to have experience and high professional qualifications. If not detected and treated quickly, the mother can go into shock due to blood loss, endangering her life, and the fetus in the uterus can easily miscarry or lose its heartbeat.

Fortunately, the patient was accurately diagnosed and promptly operated on, ensuring the mother's safety and keeping the fetus in the uterus.

Thanks to the timely intervention and solid expertise of the doctors at the Central Maternity Hospital, Ms. T. was saved and the remaining fetus was kept in the uterus.

Currently, the fetus is developing well at week 9, the fetal heartbeat is clear - a hopeful result after many losses and a long journey to find a child.

Doctors recommend that women who become pregnant after assisted reproductive methods (ovulation stimulation, IUI, IVF, etc.) need to be closely monitored and have early ultrasounds. Ovarian stimulation can lead to the risk of multiple pregnancies or ovarian hyperstimulation, ectopic pregnancy, etc., so they need to be closely monitored to detect abnormalities early.

Cases of pregnancy with ectopic pregnancy need special attention, easily confused with ovarian hyperstimulation, so differential diagnosis is extremely important. When experiencing abdominal pain in the early stages of pregnancy, the patient should immediately go to a reputable obstetrics facility for examination, diagnosis and timely treatment.
