Improving the quality of primary health care after the merger

Grassroots health care has long been considered the “gatekeeper” protecting people’s health. After merging with commune-level administrative units, commune and ward health stations quickly stabilized their organization and performed well in primary health care. At the same time, they strengthened preventive health care, continuing to affirm their role in protecting and caring for people’s health.

The core objective of the Tuyen Quang Health sector is to ensure that medical examination and treatment services are not interrupted after the merger. After the merger, the Department of Health advised the Provincial People's Committee to issue a decision regulating the functions, tasks and powers of the Health Centers under the Department of Health. At the same time, the Department of Health also issued a document directing the Health Centers of the regions to carry out tasks related to the arrangement of health stations according to the new commune-level administrative units.

According to Dr. Le Dao Bich, Head of the Planning and Finance Department, Department of Health, up to now, the whole province has 24 regional polyclinics, 100 new commune health stations and 203 health stations. Maintaining health stations has contributed to reducing difficulties in geographical distance and population dispersion. Health stations and basic stations are capable of performing initial medical examination and treatment, managing people's health and implementing preventive health programs in the community.

In fact, after the merger, grassroots health care activities not only remained stable but were also enhanced with preventive health activities and periodic health care for priority groups organized. For example, at the end of October 2025, Bac Me Regional Health Center coordinated to organize leprosy and dermatological examinations for people in Minh Ngoc, Duong Hong, and Bac Me communes; Meo Vac Regional Health Center coordinated with commune health stations to implement periodic health examinations for the elderly in Meo Vac, Sung Mang, Khau Vai, Tat Nga, and Son Vi; Vi Xuyen Regional Medical Center and local health stations organize communication and education activities to raise awareness about adolescent health care at local secondary and high schools... These activities contribute to enhancing communication and bringing quality medical services closer to people, helping to reduce the burden of travel costs, especially for disadvantaged groups and patients in remote and scattered areas.

Dr. La Dang Tai, Deputy Director of the Department of Health, said: In order to improve the quality of primary health care, the Department of Health also focuses on training and fostering professional skills. For example, the class of Commune Health Stations operating on the principle of family medicine is currently being opened with 162 students who are doctors and medical staff of commune and ward health stations participating. In November, the Department of Health also organized 5 training courses for 288 population officers and collaborators. Through the training courses, grassroots health workers have improved their management capacity as well as their ability to provide comprehensive personal and community health care. From there, the effectiveness of primary care and medical examination and treatment has been gradually improved, contributing to reducing the burden on upper levels.

Although many synchronous solutions have been implemented and maintained, difficulties in human resources, facilities as well as synchronization in the management of the grassroots health system after the merger are also issues currently being raised. These barriers require the health sector as well as local authorities to have synchronous solutions from investment in facilities, equipment to policies to attract human resources and standardize management processes. Thereby, creating conditions for grassroots health to develop, worthy of the position of "gatekeeper" protecting people's health.
