On March 7th, Dr. Nguyen Minh Tien, deputy director of the Children's Hospital of Ho Chi Minh City, announced that they had successfully saved the life of a one-year-old boy, NVTP, from Hau Giang province, who suffered a rare tracheal rupture and multiple rib fractures after falling from his bed onto the floor.
The medical history revealed that the child was playing on the bed when he fell onto the floor in a supine position. After the fall, the child cried but was alert, did not vomit, and did not have seizures. On the second day, the family noticed swelling in the child's neck and chest, so they took him to the local hospital for examination and admission.
At the hospital, doctors noted that the child was lethargic, had cyanotic lips, difficulty breathing, an SpO2 of 85%, and was intubated, placed on a ventilator, and underwent a CT scan of the head, chest, and abdomen.
The scan results showed that the child had subcutaneous emphysema in the abdomen, hips, back, chest, and neck on both sides; collapse of the upper lobes of both lungs and the middle lobe of the right lung; tracheal rupture; mediastinal emphysema; and fractures of ribs 8, 9, and 10...
Baby P. was transferred to the City Children's Hospital and underwent a multidisciplinary consultation involving respiratory, ENT, surgical intensive care, and anesthesiology specialists. The diagnosis was tracheal rupture and fractures of ribs 8, 9, and 10 (right). The doctors unanimously agreed on a treatment plan involving open-chest surgery for tracheal reconstruction, combined with bronchoscopy during the operation.
After surgery, the child was transferred to the surgical intensive care unit for further treatment with respiratory support, antibiotics, intravenous fluids, pain relievers, sedatives, and electrolyte and acid-base balance correction.
After nearly two weeks of treatment, the child's condition improved, the mediastinal and subcutaneous emphysema gradually resolved, and the child was weaned off the ventilator and the pleural drainage tube was removed. The child was alert and breathing ambient air.
According to Doctor Minh Tien, the case of patient P. is a rare case of tracheal rupture in children.
This case highlights the importance of always having someone supervise children under 3 years old, as children at this age often explore the world around them by crawling, touching strange objects, and putting them in their mouths, leading to unfortunate accidents (burns, electric shocks, poisoning from accidentally ingesting chemicals or medications, injuries from falls, etc.).
