Sepsis from unexpected habits

Recently, the Department of Gastrointestinal Infectious Diseases, Institute of Clinical Infectious Diseases, Military Central Hospital 108, admitted a male patient born in 2005 from Hanoi .

Before being admitted to the hospital, the patient had a pimple on the left cheek of the lower lip, accompanied by swelling, redness, and warmth. The patient had squeezed the pimple themselves, after which they developed a fever of 38 degrees Celsius, chills, and shivering. They self-medicated with fever reducers at home, but their condition did not improve.

The family transferred the patient to Military Central Hospital 108 in a critical condition with a high fever of 38-38.5 degrees Celsius, chills, dry lips; swelling and pus discharge in the left cheek area, limited mouth opening; mild shortness of breath, chest pain, and oxygen support via nasal cannula at 3 liters/minute.

The patient underwent tests and was diagnosed with: Staphylococcus aureus sepsis entering from a left facial-chin abscess, with a secondary focus of bilateral necrotizing pneumonia.

Acne (commonly known as pimples) is a common skin condition affecting people of all ages.

Sebum, dead skin cells, and bacteria trapped in pores are the cause of pimples, which appear as red, tender bumps with white pus at the tip.

When pimples form, pores swell and are under a lot of pressure. However, squeezing pimples can disrupt the skin's structure, causing more widespread inflammation and infection, destroying the localized inflammatory zone, and leading to the formation of other pimples around them.

In particular, there is an area on the face called the danger triangle, which can be identified by placing your hand so that the tip of your middle finger touches the nasal bone, with your palm covering the entire nose, mouth, and chin area.

This area has many veins connecting the nerves in the cranial region, which help transport blood to the brain.

Normal veins have valves to prevent blood from flowing backward; however, the veins in this area are valveless.

Inflammation in the triangular area can lead to diseases such as sepsis, meningitis, facial muscle paralysis, facial nerve damage causing facial paralysis, and even death.

According to Colonel Dr. Nguyen Dang Manh, Director of the Institute of Clinical Infectious Diseases, sepsis is a systemic infection and intoxication caused by the invasion of bacteria and bacterial toxins into the bloodstream.

In septicemia, many dangerous complications often arise, especially septic shock and multiple organ failure, with a high mortality rate.

Most types of bacteria that cause disease in humans can lead to septicemia; however, some groups of Gram-negative bacteria are commonly encountered, including: Salmonella, Escherichia coli, Klebsiella, Serratia, Pseudomonas aeruginosa, Burkholderia pseudomallei, and A. baumannii.

Gram-positive bacteria: Streptococcus pneumoniae, Staphylococcus aureus, Streptococcus suis… Anaerobic bacteria: Clostridium perfringenes and Bacteroides Fragilis. Therefore, any injury to the body can cause infection and carry the risk of sepsis.

According to doctors, to prevent sepsis, people need to implement hygiene and workplace safety measures and promptly treat any infections.

Do not pop pimples yourself. Monitor and treat chronic diseases such as diabetes, cirrhosis, etc., to reduce the risk of infection.

Minimize procedures, ensure aseptic techniques are followed, and implement standard precautions. Avoid overuse of corticosteroids and use antibiotics only as prescribed.
