On August 21st, news from Thanh Vu Medic General Hospital in Bac Lieu reported that doctors at the hospital had successfully treated a female patient with end-stage renal failure and maxillary sinusitis that had caused life-threatening complications.

The patient, Ms. N. (23 years old, residing in Thanh Tri District, Soc Trang Province ), has a history of diabetes since the age of 7. By the age of 18, she was diagnosed with end-stage chronic kidney failure, requiring regular dialysis, along with multiple other conditions including hypertension, solitary thyroid nodule, and chronic sinusitis.
Recently, Ms. N.'s sinusitis worsened, causing headaches, dizziness, nasal congestion, phlegm production, and foul-smelling, thick nasal discharge. She had taken medication but it didn't help, so her family took her to Thanh Vu Medic General Hospital in Bac Lieu for treatment.
Dr. Nguyen Hong Tru, Head of the Ear, Nose, and Throat Clinic at Thanh Vu Medic General Hospital in Bac Lieu, said that upon receiving the patient, the doctor ordered paraclinical tests. The CT scan of the head revealed that the patient had a fungal infection of the left maxillary sinus, spreading and causing a perforation of the septum, invading the right nasal cavity – a very dangerous condition.
The patient is young but has multiple underlying medical conditions and requires regular hemodialysis, making the choice of surgical method very difficult due to the risk of complications such as blood clots or massive bleeding.
After consultation, the doctors agreed on a treatment plan: dialysis one day before surgery. During the surgery, the doctors adjusted the dosage of anticoagulants to prevent bleeding. Then, they opened the left maxillary sinus, discovering a large amount of pus and clumps of green fungus. The doctors immediately irrigated the maxillary sinus, disinfected, and cleaned the area.
The surgery was successful, and the patient is recovering well; they are expected to be discharged in a few days.
Dr. Nguyen Hong Tru advises that when suffering from maxillary sinus fungal infections, the fungus can invade the eye socket, causing gradual blurring of vision and potentially permanent blindness. The fungus can also cause meningitis and encephalitis if it invades the skull. Furthermore, if the fungus invades large blood vessels, it can cause massive bleeding, posing a life-threatening risk. Therefore, when experiencing unusual symptoms in the nasal and sinus areas, patients should immediately go to the hospital for examination and timely treatment to avoid future complications.
