Inter-hospital coordination of cesarean section for pregnant women with cancer

NDO - The joint surgical team of K Hospital and Central Maternity Hospital has just performed surgery to deliver a baby boy at 31 weeks, weighing 1,600 grams, from a pregnant woman with stage IVA Non-Hodgkin lymphoma.

Ms. Phan Thi Thu T. (28 years old, from Phu Tho ) is pregnant for the third time. When the pregnancy was 25-26 weeks, the patient had dull abdominal pain in the lower abdomen with vaginal bleeding.

Examination at K Hospital, the patient was diagnosed with Non-Hodgkin lymphoma manifesting in the cervix at 26 weeks of pregnancy. After careful consultation with the doctor, Ms. T. still made the decision... "I want to keep the baby".

The short sentence but containing the boundless love of a mother for the little creature that is beating the same rhythm made the doctors at K Hospital decide to discuss with the Central Maternity Hospital to closely monitor the development of Ms. T's tumor and hope to prolong the maximum number of weeks of pregnancy.

In early September 2022, Ms. T. noticed more vaginal bleeding. After examination and monitoring at the Central Obstetrics Hospital, the doctor determined that the vaginal bleeding was coming from the tumor. Doctors at the Central Obstetrics Hospital immediately discussed and transferred the patient to K Hospital for surgery.

The consultation took place on the night of September 6th, the experts from the two hospitals assessed that the tumor was large and ulcerated, occupying the entire cervix, and was bleeding heavily. Along with that, the patient was in labor, had uterine contractions, and proposed a plan to perform a cesarean section for the mother and remove the entire uterus to stop the bleeding for Ms. T. that same night.

The surgical team also included doctors from both hospitals: Dr. Pham Thi Dieu Ha, Deputy Head of the Department of Gynecological Surgery, K Hospital and Associate Professor, Dr. Nguyen Manh Thang, Central Obstetrics Hospital. The surgery was carefully considered and calculated to ensure the safety of both the mother and the fetus.

Just a few minutes after the surgery, at exactly 9 p.m., the baby boy weighing 1,600 grams was born. His cry seemed to make the surgical team burst with joy. The baby was transferred to the Neonatal Department, Central Maternity Hospital. The surgery continued to be performed smoothly despite many difficulties.

Dr. Pham Thi Dieu Ha, Deputy Head of the Department of Gynecological Surgery, said, "The risk of bleeding during surgery is very high because patient T. underwent two surgeries at the same time, the tumor in the cervix was too large and the cervical canal was also very large, erasing all anatomical boundaries. Along with that, the uterus had not yet contracted because the mother had a cesarean section, causing further loss of anatomical landmarks, causing many difficulties for us during surgery."

During the surgery, the doctor found a cervical tumor measuring 6x10cm, necrotic and bleeding, so the team removed Ms. T's entire uterus.

Due to the previous heavy blood loss, the patient was actively resuscitated and transfused with 4 units of red blood cells and 4 units of plasma. The surgery was successful thanks to the smooth coordination of the inter-hospital surgical team. Currently, the patient is conscious, responsive, and continues to be monitored and treated.

Ms. T.'s family is very happy because Ms. T. is gradually recovering and the baby is being cared for and is progressing every day. "Seeing mother and child like this every day makes my family very happy. I hope my son will grow up healthy and my wife will recover well. I have no other wish because that is the greatest wish of the whole family. Thank you very much to the doctors at the two hospitals," Ms. T.'s husband expressed.
