Almost perforated intestine due to stray IUD

VnExpressVnExpress19/08/2023

Nghe An Two women, 29 and 30 years old, went to the doctor due to many days of dull abdominal pain in the pelvic area below the navel and digestive disorders. Ultrasound detected an IUD in the abdomen, at risk of perforating the colon.

On August 19, Nghe An Obstetrics and Pediatrics Hospital said that two patients were admitted a week ago. The two had had contraceptive devices inserted at private medical facilities 5-7 months ago, but did not return for a check-up or follow-up examination, but were found to be normal.

CT scans showed that in the 29-year-old woman, the IUD had strayed into the cecal wall. In the 30-year-old patient, the IUD was located at one end in the ovary and at the other end in the serosa.

With these locations, the doctor said that if left untreated, the risk of colon perforation is very high. After several hours of endoscopy, the surgical team removed the IUDs from the ovaries and surrounding colon. The patient's health is currently stable and he has been discharged from the hospital.

An intrauterine device (IUD) is a small device (usually T-shaped) that is placed in the uterus to prevent pregnancy for many years. The two common types of IUDs today are the T-shaped and the arc-shaped, copper-wrapped IUD. The mechanism of the IUD is to cause an inflammatory reaction in the uterine lining, changing the biochemical structure of the endometrium cells, preventing the fertilized egg from implanting in the uterus.

Doctors say that IUDs are a temporary contraceptive method for women, highly effective, easy to use, economical and effective for many years. However, IUDs can cause some complications such as irregular menstruation, back pain, especially misplacement.

Ectopic IUD is a rare condition. The IUD is not in the correct position in the uterine cavity but is located in other abnormal positions such as in the uterine muscle, in the ligaments, in the abdominal cavity, or even in other abdominal organs. This condition is dangerous for the patient, as complications such as bleeding, intestinal perforation, or peritonitis may occur.

Doctors recommend removing the IUD after 5 years when it has stopped being effective. During the time the IUD is in place, regular gynecological examinations are required or immediately upon any unusual signs to detect complications and treat them promptly.
