Previously, on the morning of July 6, the emergency department of Nghe An General Hospital received patient BVN . (53 years old), residing in Nam Dan, Nghe An, in a state of severe pain around the navel, bruising, skin abrasions in the chest and abdomen, pale mucous membranes, dizziness, rapid pulse, low blood pressure, abdominal distension...
According to patient N.'s family, about 10 days before being hospitalized, the patient had a work accident due to falling from a height. Subsequently, the patient experienced prolonged abdominal pain and had previously sought treatment at another medical facility without improvement, so his family brought Mr. N. to Nghe An Friendship General Hospital for examination and treatment.
Here, the patient was quickly examined, underwent multidisciplinary consultations, and had various diagnostic tests performed. The CT scan results showed: Grade IV liver injury with vascular injury, fracture of the anterior arch of the left sixth rib, and a 10mm thick left pleural effusion.
Following a consultation, the patient was diagnosed with: Hemorrhagic shock due to grade IV liver rupture of lobe 2, blunt chest trauma – fracture of the 6th left rib, and left pleural effusion. Without timely surgery, the patient faced a very high risk of death due to rapid and massive intra-abdominal bleeding, leading to hemorrhagic shock, multiple organ failure, and circulatory arrest.
"Grade IV liver injury is one of the most severe injuries, especially when blood vessels are torn, and liters of blood can be lost in just a few minutes. Many cases, if not treated in the "golden hour" (first 1-2 hours), will not survive," said Dr. Nguyen Huy Toan.
Faced with a critical situation threatening the patient's life, the doctors and nurses at Nghe An Friendship General Hospital promptly activated the internal red alert system, quickly mobilizing a team of emergency, anesthesia, and surgical doctors, and simultaneously transferring the patient to the operating room for emergency surgery.
Under the direct guidance of Dr. Nguyen Huy Toan, the surgery lasted for many hours with smooth, urgent and precise coordination between the specialties. The doctors controlled the bleeding in the abdomen and stabilized the patient's hemodynamics.
