Saving the life of a young man with a giant pancreatic pseudocyst using modern techniques

A 30-year-old man with a large pancreatic pseudocyst was saved by doctors using LAMS stent placement technique.

" Giant " cyst torments after pancreatitis

On the morning of December 9, information from Can Tho Central General Hospital said that the patient had successfully undergone intervention for a "giant" pancreatic pseudocyst using the technique. LAMS stent placement ultrasound endoscopy is Mr. PTN (30 years old, in Can Tho city).

Previously, Mr. N. was hospitalized with diffuse abdominal pain, mild abdominal distension, and continuous vomiting. The patient had a history of pancreatitis that lasted for many years and had to be hospitalized frequently for treatment, affecting his quality of life.

Through the results of the computed tomography (CT) scan, doctors discovered a cystic lesion located next to the pancreas, close to the stomach wall. The cyst was very large, up to 9 x 11 cm, causing compression, thickening the colon wall and dilating the small intestine loops, causing fluid to accumulate.

After a multidisciplinary consultation, the team of doctors decided not to perform open surgery but to choose the most optimal option of draining the pseudocyst using the LAMS stent technique under the guidance of an endoscopic ultrasound (EUS). Thanks to the ability to observe lesions at high resolution of the EUS system, the doctors accurately located the location that needed intervention. The doctor used a specialized needle to puncture the cyst, inserted a guide wire and used a cutting instrument (cytotome) to open the way into the cyst wall.

Immediately after that, the LAMS (Lumen-Applying Metal Stent) stent was inserted and deployed, creating a direct path between the cyst and the stomach. As soon as the stent opened, the lemon-yellow cyst fluid was immediately decompressed and flowed out. The entire intervention process took place quickly in about 20 minutes. By the morning of December 9, the patient was completely awake, with pink skin and mucous membranes, a soft abdomen, significantly reduced pain, and stable vital signs.

Minimally invasive, helping patients recover quickly

Specialist 2 Doctor Nguyen Khac Nam, Deputy Head of the Department of General Surgery, said that pancreatic pseudocyst is a common complication after acute or chronic pancreatitis. Common symptoms include epigastric pain, nausea, vomiting, weight loss, possibly accompanied by fever, jaundice or signs of compression of nearby organs. If not treated promptly, this pancreatic cyst can cause infection, bleeding, intestinal obstruction, or even rupture into the abdominal cavity, threatening life.

After intervention with endoscopic ultrasound and LAMS stent placement, the patient recovered.

Meanwhile, specialist doctor 2 Nguyen Thi Quynh Mai, Head of Endoscopy Department, Can Tho Central General Hospital emphasized: The application of LAMS stent under EUS guidance is a remarkable step forward. Compared to open surgery, this method is less invasive, helps to reduce pain to the maximum, shortens recovery time and limits post-operative complications. The success of the above intervention not only saved the life of a young patient with pancreatitis but also opens up the opportunity for highly effective treatment for patients with complex pancreaticobiliary diseases in the West.
