For the first time in Vietnam, using 4K endoscopy system to operate on giant pituitary tumor

(Dan Tri) - The first surgery for a giant pituitary tumor using the 4K - ICG neuroendoscopic surgery system in Vietnam was performed at Cho Ray Hospital.

On December 4, Cho Ray Hospital announced that the person who underwent surgery was Mr. NTP (57 years old, residing in Khanh Hoa province), who had a giant pituitary tumor and severely impaired vision.

Sharing with the treating doctor, the patient said that he had previously visited many places for examination and treatment for 10 years because of prolonged blurred vision, but the cause was not found to be from the nervous system.

On November 10, the patient was transferred to Cho Ray Hospital with severe headaches, pituitary failure, and very weak vision, only able to distinguish palm shadows.

At the Department of Neurosurgery, MRI results showed that the patient had a 56mm pituitary tumor (belonging to the giant pituitary tumor group, accounting for only 10% of all pituitary tumors) invading the sphenoid sinus, cavernous sinus, compressing the optic chiasm and two internal carotid arteries.

This is a very complex pathology, with the risk of damaging many important vascular and nerve structures. Notably, the rate of complete tumor removal according to medical reports recorded at major centers around the world is only 3-40%.

Faced with that situation, the treatment team decided to perform endoscopic surgery through the nose - sphenoid sinus. In particular, in this case, the team used the 4K - ICG neuroendoscopic surgery system, a modern technology that Cho Ray - the first medical facility in Vietnam - has applied since the middle of this year.

The endoscope system is equipped with specialized functions for neurosurgery, with a compact endoscope design, shortened length, mainly using rigid tubes and multi-angle viewing angles.

Dr. Tran Thien Khiem, Department of Neurosurgery of Cho Ray Hospital, said that in the case of a giant tumor like the patient above, the most important requirement is to remove as much of the tumor as possible while still preserving the healthy pituitary gland and the surrounding blood vessel and nerve system.

With the new system, when injecting ICG fluorescent agent, blood vessels and important structures will be clearly visible on the 4K screen, helping surgeons in each operation more conveniently. Thanks to that, the team can remove almost the entire tumor, increasing the effectiveness of surgery and improving the quality of treatment for patients.

Five days after surgery, the patient was alert, able to eat and drink by himself, had stable pituitary gland function, significantly improved vision, and was discharged from the hospital.

Dr. Tran Huy Hoan Bao, Head of the Department of Neurosurgery, emphasized that giant pituitary tumors and complex skull base tumors are always a big challenge in the field of neurosurgery. Tumors often invade deeply and are closely related to many important vascular and nerve structures.

With previous methods, removing the maximum amount of tumor while preserving important structures required high technique and had many potential risks of complications.

Since the 4K - ICG neuroendoscopic surgery system was put into use, Cho Ray Hospital has successfully treated more than 30 cases of complex tumors, helping to increase the safety and effectiveness of surgery.

"The implementation of this surgical system is one of the important steps forward, opening up more treatment opportunities for patients with severe and complex neurological diseases," affirmed the Head of the Department of Neurosurgery, Cho Ray Hospital.
